摘  要:目的 :观察干扰素联合羟基脲治疗慢性粒细胞白血病继发血小板增多症的疗效。方法 :将 69例随机分为五组 ,分别予羟基脲 (1~ 3 g/d)联合 α-干扰素 3 0 0万单位 ,每周 2~ 3次 ,肌肉注射 ;或联合γ-干扰素 1 0 0万单位 ,隔月 1次 ,肌注 ;或联合 α-干扰素 3 0 0万单位及 γ-干扰素 1 0 0万单位 ,用法同前 ;或联合高三尖杉酯碱 2 mg加入生理盐水 5 0 0 ml中静脉滴注 ,每周 1~ 2次 ,累计平均 65 mg;化疗组采用DA方案。结果 :羟基脲合用 α、γ-干扰素疗效最好 ,治疗前后血小板计数下降最明显 (P<0 .0 5 ) ,在减少巨核细胞数量、改善骨髓纤维化和 Bcr/ab L融合基因方面均优于其他治疗组 (P<0 .0 5 )。结论 :α、γ-干扰素联合羟基脲三者合用适用于治疗慢性粒细胞白血病继发血小板增多症。Objective: To observe the therapeutic efficacy of the treatment of chronic myelocytic leukemia cases complicated with thrombocythemia treated with interferon and hydroxyurea.Methods:66 chronic myelocytic leukemia cases complicated with thrombocythemia were divided at random into 5 groups and treated with hydroxyurea(1-3g/d) combined with α interferon(3 million units,2 to 3 times a week) i.m;or combined with 1 million γ interferon units given every other month;or combined with a interferon 3 million units and 1 million r interferon units given as the formers;or combined with homoharringtonine 2 mg in 500 ml saline intravenously,1 to 2 times a week, to a total of 65 mg;Chemotherapy group with DA.Results: The best therapeutic efficacy was seen in hydroxyurea combined with α,γ interferon group and the platelet number was reduced significantly before and after the treatment( P <0.05).It is also better in reductin macropgage numbers,improve bone marrow fibrosis and Bcr/abL fusion gene than other groups( P <0.05). Conclusion:The combined use of hydroxyurea and α,γ interferon was effective in treating chronic myelocytic leukemia cases complicated with thrombocythemia.
