Analyzing Assay Result of Serum Immunoglobulin in 24 Patients with Multiple Myloma

摘  要:收集多发性骨髓瘤(multiplemyloma,MM)患者24例,血清标本通过免疫散射比浊法测定免疫球蛋白(immunoglobulin,Ig),同时与其蛋白定量、蛋白电泳结果相比较,发现免疫球蛋白的结果与前两者检测结果吻合率为417%,不完全支持诊断分析结果表明:抗原抗体的活性、效价及最适浓度比是影响免疫反应的重要因素;因Ig存在不同种类、亚类,加之M蛋白是单克隆Ig,所用试剂对M蛋白的检测是否完全适用有待探讨;M蛋白往往无抗体活性,有自己独特抗原决定簇,也可能是造成多发性骨髓瘤患者血清中Ig检测不高的原因结果提示:Ig的检测必须充分考虑各种影响因素;Collected were 24 cases of multiple myloma (MM), whose serum samples were determined through turbidimetry for immunoglobulin (Ig), which was compared with protein in quatitive analysis and with protein electrophresis, resulting in a tally rate of 41.7%, which gave no support to the diagnosis. The results indicated that the activity, valence and optimum concentration rate of antigen and antibody are the factors affecting immune reaction. Because immunoglobulins are different in species or subspecies and M proteins are monoclonal immunoglobulin, whether the used reagents are suitable or not still remains to be studied. M proteins usually have no antibody activity, but they have their unique antigen determinants. This might be the agent which causes the low Ig detection rate in the serum sample from multiple myloma patients. The results suggest that various factors should be fully taken into account for Ig detection and M protein detection cannot be definite until tests are supplemented for some other disease cases.
