摘　　要：目的:验证短程小剂量奥氮平联合氟酉汀快速控制抑郁、焦虑的疗效及安全性。方法:34例心理咨询初诊患者符合ICD-10抑郁障碍,Hamilton抑郁量表(HAMD)17项得分≥16,同时伴发心血管、消化、泌尿、睡眠障碍等多系统躯体症状,未经抗抑郁治疗,予以奥氮平2.5mg qn×2周,合用氟西汀20mg qd×4周,治疗前和治疗后3天、1周、2周、4周分别对HAMD、HAMA(Hamilton焦虑量表)和躯体症状进行评定、观察该治疗方法起效和控制症状所需要的时间,并对其安全性进行评估。结果:短程小剂量奥氮平联合氟西汀确实具有显著快速控制抑郁、焦虑及伴发的躯体症状的作用,3天内HAMD量表减分率达33.53％,HAMA量表减分率达35.95％,4周后分别达到52.24％和55.28％,改善躯体症状的总有效率达88.41％。结论:证明了联合用药起效快、疗效好、安全性高,并可以减少单用氟西汀治疗过程中产生的一过性焦虑和消化道反应。Objective: To evaluate the efficacy and safety of low dosage of olanzapine combined with fluoxetine in the treatment of depression and anxiety. Methods: 34 depressive patients accompanied by multi - systemic dysfunction such as cardiovascular sys-tem. digestive system and urinary system were enrolled in the study, according to ICD - 10 and the rating scale of HAMD( ≥16).They didn' t undergo the anti - depressive treatment before. The treatment dose of olanzapine was 2. 5mg, qN, and fluoxetine was 20mg, qd. The patients were treated with olanzapine for 2 weeks, then with fluoxetine 4 weeks. The rating scale of HAMD and HAMA were adopted to evaluate the efficacy of antidepressants and the effective time needed to control the signs. Results: The short - term of low dosage of olanzapine combined with fluoxetine could indeed to control depression . anxiety and the accompanied signs rapidly. The decreasing rate of the scale of HAMD was 33.53% within 3 days, 35.95% within 2 weeks, 52.24% and 55.28% after 4 weeks, respectively. The total effective rate of improving the signs was 88.41% . Conclusion: The drug combination had a short effective time, a good curative effect, and high efficacy, also could decrease the spontaneously anxiety and the digestive reaction in the treatment.
