V型斜视的临床分析

摘　　要：目的 :分析V型斜视发病率、病因及探讨V型斜视不同术式的治疗效果。方法 :统计了 4 82例斜视患者 ,分析了V型斜视的病因 ,治疗V型斜视采用水平直肌附着点垂直移位和 (或 )下斜肌后徙术。结果 :① 4 82例斜视中 ,有 6 2例V型斜视 ,占 12 8% ;② 6 2例V型斜视中仅有下斜肌功能亢进者 2 2例 ,下斜肌功能亢进 +垂直肌功能异常者 9例 ,下斜肌功能亢进 +水平肌功能异常者 7例 ,下斜肌功能亢进 +水平肌功能异常 +垂直肌功能异常者 3例 ,仅有水平肌功能异常者 18例 ;水平肌肉附着点垂直移位者 3例。③两种术式效果无显著差异。结论 :①V型斜视在斜视中不少见 ,不要漏诊。②产生V型斜视的因素较多 ,多数V型斜视都有下斜肌功能亢进。Objective:To study the incidence,pathogeny,the effect with different surgical methods of V-strabismus.Method:four hundred eighty two patients with strabismus were studied.The pathogeny of each V-strabismus case was analyzed.Vertical transportation of horizontal retus insertions and(or)recession of inferior oblique muscles were performed to correct V-strabismus.Result:①The incidence of V-strabismus was 12 8% in this group;②22 cases had only excited function of inferior oblique muscles,9 cases had excited function of inferior oblique muscles and abnormal function of vertical rectus,7 cases had excited function of inferior oblique muscles and abnormal function of horizontal rectus,3 cases had excited function of inferior oblique muscles,abnormal function of vertical rectus and horizontal rectus,18 cases had only abnormal function of horizontal rectus,and 3 cases had vertical transportation of horizontal rectus insertion.The proportion of excited function of inferior oblique muscles was 66 1% and③The effect of the two surgical methods gave no significant differences.Conclusions:①V-strabismus was not common,one should never miss the diagnosis,②There were several factors influencing the occurrence V-strabismus.The majority of V-strabismus had excited function of inferior oblique muscles,and③Two surgical methods used were both effective.
