摘  要:目的分析血清信号肽-CUB-表皮生长因子结构域包含蛋白1(SCUBE1)、C型凝集素样受体-2(CLEC-2)、解偶联蛋白2(UCP2)对急性缺血性脑卒中(AIS)患者静脉溶栓(IVT)后预后不良的预测价值。方法选取2022年6月至2023年10月在该院接受IVT治疗的116例AIS患者为观察组,另选取同期在该院进行体检的116例健康者为对照组。根据改良Rankin量表(mRS)评分将患者分为预后良好组和预后不良组。采用酶联免疫吸附试验检测血清SCUBE1、CLEC-2、UCP2水平。采用多因素Logistic回归分析AIS患者IVT后预后不良的影响因素。绘制受试者工作特征(ROC)曲线分析血清SCUBE1、CLEC-2、UCP2对AIS患者IVT后预后不良的预测价值。结果观察组血清SCUBE1、CLEC-2水平高于对照组,血清UCP2水平低于对照组,差异均有统计学意义(P<0.05)。预后良好组有75例患者,预后不良组有41例患者。预后不良组梗死面积>4 cm~3患者比例、血清SCUBE1、CLEC-2水平高于预后良好组,血清UCP2水平低于预后良好组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,血清SCUBE1、CLEC-2水平升高为AIS患者IVT后预后不良的危险因素(P<0.05),血清UCP2水平升高为AIS患者IVT后预后不良的保护因素(P<0.05)。ROC曲线分析结果显示,血清SCUBE1、CLEC-2、UCP2单独预测AIS患者IVT后预后不良的曲线下面积(AUC)分别为0.752、0.694、0.776,3项指标联合预测的AUC为0.861,3项指标联合预测的AUC大于SCUBE1、CLEC-2、UCP2单独预测的AUC(Z_(3项联合-SCUBE1)=2.358、Z_(3项联合-CLEC-2)=2.714、Z_(3项联合-UCP2)=2.591,P=0.018、0.007、0.010)。结论IVT后预后不良的AIS患者血清SCUBE1、CLEC-2水平升高,UCP2水平降低,3项指标联合检测对AIS患者IVT后预后不良有较高的预测价值。Objective To analyze the predictive value of the serum signal peptide-cub epidermal growth factor domain-containing protein 1(SCUBE1),C-type lectin like receptor-2(CLEC-2)and uncoupling protein 2(UCP2)in patients with acute ischemic stroke(AIS)after intravenous thrombolysis(IVT).Methods A total of 116 AIS patients who received IVT treatment at this hospital from June 2022 to October 2023 were collected as the observation group,and 116 healthy individuals who underwent physical examinations in this hospital during the same period were regarded as the control group.According to the modified Rankin Scale(mRS)score,the patients were classified as the good prognosis group and the poor prognosis group.Enzyme linked immunosorbent assay was used to determine the levels of serum SCUBE1,CLEC-2 and UCP2.Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in AIS patients after IVT.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum SCUBE1,CLEC-2 and UCP2 for poor prognosis in AIS patients after IVT.Results The serum levels of SCUBE1 and CLEC-2 in the observation group were higher than those in the control group,and the serum level of UCP2 in the observation group was lower than that in control group,with statistical significance(P<0.05).There were 75 patients in the good prognosis group and 41 patients in the poor prognosis group.The proportion of infarct area>4 cm 3,serum levels of SCUBE1 and CLEC-2 in the poor prognosis group were higher than those in the good prognosis group,and serum level of UCP2 in the poor prognosis group was lower than that in the good prognosis group,with statistical significance(P<0.05).Multivariate Logistic regression analysis showed that the increase of serum SCUBE1 and CLEC-2 levels was a risk factor for poor prognosis in AIS patients after IVT(P<0.05),and the increase of serum UCP2 level was a protective factor for poor prognosis in AIS patients after IVT(P<0.05).ROC curve analysis results showed that the ar
