摘 要:目的:分析基于预后营养指数(PNI)的营养干预对乙肝失代偿期肝硬化(DHBC)患者胃肠激素水平及营养状态的影响。方法:选取郑州市第三人民医院2021年10月至2023年10月收治的87例乙肝DHBC患者临床资料,依照护理干预模式不同分为两组,对照组42例接受常规护理干预,观察组45例在对照组基础上接受基于PNI的营养干预,比较两组干预前后胃肠激素(胃动素、胃泌素)水平、营养状态[血清白蛋白(ALB)、运铁蛋白(TRF)、总蛋白(TP)、血红蛋白(Hb)]、肝功能[血清谷草转氨酶(AST)、总胆红素(TBil)、谷丙转移酶(ALT)]、生活质量[慢性肝病问卷(CLDQ)]。结果:干预后,两组胃动素、胃泌素水平均降低,且观察组低于对照组(P<0.05);干预后,两组血清ALB、TRF、TP、Hb水平均升高,且观察组升高幅度更大(P<0.05);干预后,观察组血清AST、TBil、ALT水平均低于对照组(P<0.05);干预后,两组生活质量均提升,且观察组CLDQ各维度评分均较对照组高(P<0.05)。结论:基于PNI的营养干预应用于乙肝DHBC患者,能改善胃肠激素水平、营养状态、肝功能,促使生活质量提升。Objective:To analyze the effect of nutritional intervention based on the prognostic nutritional index(PNI)on gastrointestinal hormone levels and nutritional status in patients with decompensated hepatitis B cirrhosis(DHBC).Methods:Clinical data of 87 patients with hepatitis B DHBC admitted to the Third People's Hospital of Zhengzhou from October 2021 to October 2023 were collected and divided into two groups based on different nursing intervention models.The control group(42 patients)received routine nursing intervention,while the observation group(45 patients)received PNI-based nutritional intervention in addition to the routine nursing provided to the control group.The levels of gastrointestinal hormones(motilin and gastrin),nutritional status[serum albumin(ALB),transferrin(TRF),total protein(TP),hemoglobin(Hb)],liver function[serum aspartate aminotransferase(AST),total bilirubin(TBil),alanine aminotransferase(ALT)],and quality of life[chronic liver disease questionnaire(CLDQ)]were compared between the two groups before and after the intervention.Results:After the intervention,the levels of motilin and gastrin decreased in both groups,with lower levels observed in the observation group compared to the control group(P<0.05).The levels of serum ALB,TRF,TP and Hb increased in both groups after the intervention,with a more significant increase observed in the observation group(P<0.05).The levels of serum AST,TBil and ALT were lower in the observation group compared to the control group after the intervention(P<0.05).The quality of life improved in both groups after the intervention,and the scores of each dimension of CLDQ were higher in the observation group compared to the control group(P<0.05).Conclusion:Nutritional intervention based on the PNI for patients with DHBC can improve gastrointestinal hormone levels,nutritional status,liver function,and promote an improvement in quality of life.
