网织红细胞RET、IRF、HFR在肿瘤患者发病不同时期的临床意义

摘  要:目的通过观察肿瘤患者不同病程网织红细胞RET%、RET#、IRF、HFR%、HFR#5项参数的变化,评估肿瘤患者放化疗后骨髓造血功能以及变化趋势。方法对2023年1月—12月入住南通瑞慈医院肿瘤科患者的临床特征进行统计,观察恶性肿瘤的构成比。采集EDTA-K2抗凝血检测RET%、RET#、IRF、HFR%、HFR#,分析这些参数在肿瘤早中晚期的差异性。将接受放化疗5次及以上的肿瘤患者样本按肿瘤部位分组,分析各组参数的变化趋势。结果912例肿瘤患者中,排名前5位的肿瘤分别是肺癌154例(16.86%)、乳腺癌128例(14.00%)、肝癌85例(9.31%)、胃癌60例(6.57%)和胰腺癌59例(6.46%)。A组的恶性肿瘤中构成比最高的是血液肿瘤(24.46%),其次为肝癌(19.20%)。B组中构成比最高为肺癌(25.53%);其次为消化系统肿瘤(食管癌、胃癌和结直肠癌)。网织红细胞5项参数检测结果表明,748例B组样本测得的RET%、RET#、IRF、HLR%、HLR#5项参数的均值均高于349例A组样本,A、B两组5项参数的偏倚度分别为+15.92%、+7.41%、+9.34%、+32.83%、+39.62%。各系统肿瘤在不同分期及治疗阶段的网织红参数呈现出不同趋势,肝癌、胰腺癌均上升,食管癌、胃癌、肠癌基本无变化。结论网织红细胞RET%、RET#、IRF、HLR%、HLR#5项参数的变化对评估肿瘤患者放化疗后骨髓红细胞系统造血功能、疗效和预后具有一定临床价值。Objective To assess the recovery and trend of bone marrow hematopoietic function in tumor patients after radiochemotherapy by observing the changes in reticulocyte parameters RET%,RET#,IRF,HFR%,and HFR#during different stages of tumor onset.Methods Clinical characteristics of tumor patients admitted to the Oncology Department of Nantong Ruici Hospital from January 2023 to December 2023 were statistically analyzed to observe the composition ratio of malignant tumors.EDTA-K2 anticoagulated blood was collected to test RET%,RET#,IRF,HFR%,and HFR#,and the differences in these parameters in the early,middle,and late stages of tumors were analyzed.Samples from tumor patients who had undergone radiochemotherapy five times or more were grouped by tumor site to analyze the trend of changes in each group’s parameters.Results Among the 912 tumor patients,the top five types of tumors were lung cancer with 154 cases(16.86%),breast cancer with 128 cases(14.00%),liver cancer with 85 cases(9.31%),gastric cancer with 60 cases(6.57%),and pancreatic cancer with 59 cases(6.46%).In Group A,the highest composition ratio of malignant tumors was hematological tumors(24.46%),followed by liver cancer(19.20%).In Group B,the highest composition ratio was lung cancer(25.53%),followed by digestive system tumors(esophageal,gastric,and colorectal cancers).The results of the five reticulocyte parameter tests showed that the mean values of RET%,RET#,IRF,HLR%,and HLR#in 748 samples from Group B were all higher than those in 349 samples from Group A.The biases of the five parameters in Groups A and B were+15.92%,+7.41%,+9.34%,+32.83%,and+39.62%,respectively.The reticulocyte parameters of tumors in different systems showed different trends at different stages and treatment phases,with increases in liver and pancreatic cancers,and no significant changes in esophageal,gastric,and colorectal cancers.Conclusions The changes in the five reticulocyte parameters RET%,RET#,IRF,HLR%,and HLR#have certain clinical value in assessing the hematopoietic function
