原发性闭角型青光眼手术患者术后发生恶性青光眼的影响因素

摘　　要：目的:分析原发性闭角型青光眼(PACG)手术患者术后发生恶性青光眼的影响因素。方法:选取2021年10月至2023年10月该院84例拟行手术治疗的PACG患者进行横断面研究,统计PACG手术患者术后恶性青光眼发生情况,采用Logistic回归分析PACG手术患者术后发生恶性青光眼的影响因素。结果:84例PACG手术患者术后发生恶性青光眼25例,占29.76%,设为发生组,未发生恶性青光眼59例(70.24%),设为未发生组;发生组年龄<60岁,眼轴长度<22 mm、术前高眼压、晶状体厚度<4.5 mm、前房深度<2 mm、房角结构完全关闭等占比均高于未发生组,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,年龄<60岁,眼轴长度<22 mm、术前高眼压、晶状体厚度<4.5 mm、前房深度<2 mm、房角结构完全关闭等均为影响PACG手术患者术后发生恶性青光眼的危险因素(OR>1,P<0.05)。结论:年龄<60岁,眼轴长度<22 mm、术前高眼压、晶状体厚度<4.5 mm、前房深度<2 mm、房角结构完全关闭均为影响PACG手术患者术后发生恶性青光眼的危险因素。Objective:To analyze influencing factors of malignant glaucoma after primary angle-closure glaucoma(PACG)surgeries.Methods:A cross-sectional study was conducted on 84 patients with PACG who were scheduled to undergo surgical treatment in this hospital from October 2021 to October 2023.The incidence of malignant glaucoma after PACG surgeries was statistically analyzed.Logistic regression was used to analyze the influencing factors of malignant glaucoma after PACG surgeries.Results:There were 25 cases of malignant glaucoma in the 84 patients with PACG surgeries,accounting for 29.76%,they were set as the occurrence group,and 59 cases(70.24%)without malignant glaucoma were set as the non-occurrence group.The proportions of the patients with age<60 years old,axial length<22 mm,preoperative high intraocular pressure,lens thickness<4.5 mm,anterior chamber depth<2 mm,and complete closure of angle structure in the occurrence group were higher than those in the non-occurrence group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that age<60 years old,axial length<22 mm,preoperative high intraocular pressure,lens thickness<4.5 mm,anterior chamber depth<2 mm,and completely closed chamber-angle structure were all risk factors for malignant glaucoma after PACG surgeries(OR>1,P<0.05).Conclusions:Age<60 years old,axial length<22 mm,preoperative high intraocular pressure,lens thickness<4.5 mm,anterior chamber depth<2 mm,and completely closed chamber-angle structure are the risk factors for malignant glaucoma after PACG surgeries.
