摘 要:目的:观察内镜碘染色、颜色定量分析及放大内镜窄带成像技术(ME-NBI)联合检查在早期食管癌诊断中的效能。方法:回顾性分析2020—2023年江阴市人民医院收治的86例疑似早期食管癌患者的临床资料,均行内镜碘染色、颜色定量分析及MENBI检查,以病理检查结果作为“金标准”,统计三种方法单项及联合检查诊断早期食管癌的结果,分析其与病理检查结果的一致性,比较三种方法单项及联合检查在早期食管癌诊断中的效能。结果:病理检查结果显示,86例疑似早期食管癌患者中,阳性26例,阴性60例;内镜碘染色检查诊断阳性26例,阴性60例,与病理检查结果的一致性较差(Kappa=0.338);颜色定量分析检查诊断阳性27例,阴性59例,与病理检查结果的一致性一般(Kappa=0.427);ME-NBI检查诊断阳性25例,阴性61例,与病理检查结果的一致性一般(Kappa=0.526);联合检查阳性31例,阴性55例,与病理检查结果的一致性较高(Kappa=0.764);联合检查诊断早期食管癌的准确度均高于内镜碘染色、颜色定量分析检查,灵敏度、阴性预测值均高于内镜碘染色、颜色定量分析及ME-NBI检查,差异有统计学意义(P<0.05);内镜碘染色、颜色定量分析及ME-NBI单项及联合检查诊断早期食管癌的特异度、阳性预测值比较,差异均无统计学意义(P>0.05)。结论:内镜碘染色、颜色定量分析及ME-NBI联合检查在早期食管癌诊断中的效能高于三者单项检查诊断。Objective:To observe efficiency of combined examination of endoscopic iodine staining,color quantitative analysis and magnifying endoscopy narrow-band imaging(ME-NBI)in diagnosis of early esophageal cancer.Methods:The clinical data of 86 patients with suspected early esophageal cancer admitted to Jiangyin People’s Hospital from 2020 to 2023 were retrospectively analyzed.All patients underwent endoscopic iodine staining,color quantitative analysis and ME-NBI examination.Using the results of pathological examination as the‘gold standard’,the results of single and combined examination of the three methods in the diagnosis of early esophageal cancer were statistically analyzed,and their consistencies with pathological results were analyzed.The efficiencies of single and combined examination of the three methods in the diagnosis of early esophageal cancer were compared.Results:The pathological examination showed that among the 86 patients with suspected early esophageal cancer,26 cases were positive,and 60 cases were negative.Endoscopic iodine staining diagnosed 26 positive cases and 60 negative cases,and the consistency with pathological examination results was poor(Kappa=0.338).Endoscopic color quantitative analysis diagnosed 27 positive cases and 59 negative cases,and the consistency with pathological examination results was fair(Kappa=0.427).There were 25 positive cases and 61 negative cases diagnosed by ME-NBI,and the consistency with the results of pathological examination was fair(Kappa=0.526).The combined examination detected 31 positive cases and 55 negative cases,which had a high consistency with the pathological examination results(Kappa=0.764).The accuracy of combined examination in the diagnosis of early esophageal cancer was higher than that of endoscopic iodine staining and color quantitative analysis;the sensitivity and negative predictive value were higher than those of endoscopic iodine staining,color quantitative analysis and ME-NBI;and the differences were statistically significant(P<0.05).Th
