摘　　要：早发性卵巢功能不全(POI)是一种育龄期女性常见的生殖内分泌疾病,以月经紊乱、血清高水平卵泡刺激素和低水平雌激素为特征,对女性身心健康构成严重威胁。大多数POI患者病因不明,可能与自身免疫因素相关,主要表现为并发自身免疫性疾病,免疫细胞功能紊乱,促炎性细胞因子与抗炎性细胞因子失衡以及卵巢自身抗体的存在。西医的治疗主要依赖于激素和辅助生殖技术,但存在许多不良反应和禁忌证。中医药的优势在于安全性极高和副作用较小,能够通过多种成分、多个靶点、多条通路调控免疫功能障碍,防治POI。未来,进一步探究免疫性POI的分子和病理机制,有望深入理解免疫功能与POI发生发展的关联,并寻找潜在的药理靶点以防治免疫性POI。Premature ovarian insufficiency(POI)is a common reproductive endocrine disease among women of childbearing age.It is characterized by menstrual disorders,elevated levels of follicle stimulating hormone and decreased levels of estrogen in serum,which poses a serious threat to women′s physical and mental health.The etiology of the majority of POI patients remains unclear,which may be related to autoimmune factors.Their main manifestations include concurrent autoimmune disorders,immune cell dysfunctions,imbalances between pro-inflammatory and anti-inflammatory factors,and the presence of ovarian autoantibodies.Western medical treatment predominantly relies on hormonal therapies and assisted reproductive technologies;however,these approaches often come with numerous adverse reactions and contraindications.Traditional Chinese medicine holds the advantages of high safety and few adverse reactions,which has shown potential in preventing and treating POI by regulating immune dysfunction through multiple components,targets,and pathways.In the future,further exploration of the molecular and pathological mechanisms of immune POI is expected to provide a deeper understanding of the association of immune function with the occurrence and development of POI and search for the potential pharmacological targets against immune POI.
