精神科护士职业倦怠与道德困境、工作投入及应对方式的相关性  

Correlation of occupational burnout with moral dilemmas,work engagement,and coping styles of psychiatric nurses

摘  要:目的探讨精神科护士职业倦怠、道德困境、工作投入、应对方式现状及职业倦怠与道德困境、工作投入、应对方式的相关性。方法选择2020年1月至2022年12月在驻马店市第二人民医院精神科从事护理工作的280名临床护士为研究对象。收集受试者的一般人口学资料,采用Maslach职业倦怠量表-服务行业版(MBI-HSS)、中文版护士道德困境量表(MDS-R)、Utrecht工作投入量表(UWES)、简易应对方式量表(SCSQ)分别评估精神科护士的职业倦怠、道德困境、工作投入、应对方式水平,并分析职业倦怠与道德困境、工作投入、应对方式的相关性。结果280名护士的MBI-HSS得分为(66.94±13.67)分,MDS-R得分为(93.14±26.74)分,UWES得分为(57.51±12.09)分,积极应对得分为(1.69±0.52)分,消极应对得分为(1.53±0.50)分。女性护士的MBI-HSS、UWES得分显著低于男性护士,积极应对得分显著高于男性护士(P<0.05);不同性别护士的MDS-R、消极应对得分比较差异无统计学意义(P>0.05)。26~39岁、≥40岁护士的MBI-HSS得分显著高于≤25岁护士,积极应对得分显著低于≤25岁护士(P<0.05);≥40岁护士消极应对得分显著高于≤25岁护士(P<0.05),26~39岁与≤25岁护士消极应对得分比较差异无统计学意义(P>0.05);不同年龄护士的MDS-R、UWES得分比较差异无统计学意义(P>0.05)。受教育程度为本科护士的MBI-HSS得分显著高于专科护士,积极应对、消极应对得分显著低于专科护士(P<0.05);不同受教育程度护士的MDS-R、UWES得分比较差异无统计学意义(P>0.05)。护师、主管护师及以上职称人员的MBI-HSS得分显著高于护士职称、UWES得分显著低于护士职称人员(P<0.05);主管护师及以上职称人员的消极应对得分显著高于护士职称人员,积极应对得分显著低于护士职称人员(P<0.05);护师与护士职称人员的消极应对得分和积极应对得分比较差异无统计学意义(P>0.05);不同职称Objective To analyze the current status of occupational burnout,moral dilemmas,work engagement,and coping styles of psychiatric nurses,and to explore their correlations.Methods The general information of 280 clinical nurses in the Psychiatric Department of Zhumadian Second People′s Hospital from January 2020 to December 2022 was collected.The Maslach burnout inventory-human services survey(MBI-HSS),Chinese version of the moral distress scale-revised(MDS-R),Utrecht work engagement scale(UWES),and simplified coping style questionnaire(SCSQ)were used to analyze the levels of occupational burnout,moral dilemmas,work engagement,and coping styles among psychiatric nurses,respectively.The correlation of occupational burnout with moral dilemmas,work engagement,and coping styles was also analyzed.Results The MBI-HSS,MDS-R,UWES,positive and negative coping scores of 280 nurses were(66.94±13.67),(93.14±26.74),(57.51±12.09),(1.69±0.52),and(1.53±0.50),respectively.Female nurses scored significantly lower on MBI-HSS and UWES scales and significantly higher on positive coping than male nurses(P<0.05).There was no significant difference in MDS-R and negative coping scores between female and male nurses(P>0.05).Nurses aged 26-39 and those aged 40 or older had significantly higher MBI-HSS scores and significantly lower positive coping scores than those aged 25 or younger(P<0.05).Nurses aged 40 or older also had significantly higher scores on negative coping than those aged 25 or younger(P<0.05).No significant difference was observed in negative coping scores between nurses aged 26-39 and those aged 25 or younger(P>0.05).The difference in MDS-R and UWES scores was not significant among nurses of different ages(P>0.05).Nurses with a bachelor′s degree scored significantly higher on the MBI-HSS scale and significantly lower on both positive and negative coping styles than those with an associate degree(P<0.05).There was no significant difference in MDS-R and UWES scores among nurses of different education degrees(P>0.05).Nurs
