胶体果胶铋联合奥美拉唑治疗消化性溃疡的效果研究

摘　　要：目的 分析奥美拉唑联合胶体果胶铋治疗消化性溃疡的应用效果。方法 80例消化性溃疡患者,随机分为联合组和单一组,每组40例。单一组采取奥美拉唑治疗,联合组采取奥美拉唑联合胶体果胶铋治疗。对比两组患者治疗效果、幽门螺杆菌清除率、复发率、炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]。结果 联合组患者治疗总有效率97.50%高于单一组的85.00%,差异有统计学意义(χ^(2)=3.914,P=0.048<0.05)。联合组患者幽门螺杆菌清除率95.00%(38/40)高于单一组的75.00%(30/40),差异有统计学意义(χ^(2)=6.275,P=0.012<0.05)。联合组治疗3个月后复发率5.00%(2/40)低于单一组的27.50%(11/40),差异有统计学意义(χ^(2)=7.440,P=0.006<0.05)。治疗后,两组患者IL-6、TNF-α、CRP水平较治疗前下降,且联合组患者IL-6(9.21±2.25)pg/ml、TNF-α(16.02±3.62)ng/L、CRP(7.05±0.52)mg/L均低于单一组的(14.02±2.63)pg/ml、(18.82±3.58)ng/L、(10.75±0.54)mg/L,差异具有统计学意义(P<0.05)。结论 针对消化性溃疡采取奥美拉唑联合胶体果胶铋治疗,有利于改善患者机体炎性指标,提高治疗效果、幽门螺杆菌清除率,降低复发率,对改善患者预后有积极作用。Objective To analyze the practical effect of colloidal bismuth pectin combined with omeprazole in the treatment of peptic ulcer.Methods 80 patients with peptic ulcer were randomly divided into a single group and a combined group,each consisting of 40 patients.The single group received omeprazole treatment,while the combined group received omeprazole combined with colloidal bismuth pectin treatment.The therapeutic effect,clearance rate of Helicobacter pylori,recurrence rate,and inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)]were compared between the two groups.Results The total effective rate of the combined group was 97.50%,which was higher than 85.00%in the single group,and the difference was statistically significant(χ^(2)=3.914,P=0.048<0.05).The clearance rate of Helicobacter pylori in the combined group was 95.00%(38/40),which was higher than 75.00%(30/40)in the single group,and the difference was statistically significant(χ^(2)=6.275,P=0.012<0.05).The recurrence rate was 5.00%(2/40)after 3 months of treatment in the combined group,which was lower than 27.50%(11/40)in the single group,and the difference was statistically significant(χ^(2)=7.440,P=0.006<0.05).After treatment,the levels of IL-6,TNF-αand CRP in both groups were decreased compared with those before treatment;the combined group had IL-6 of(9.21±2.25)pg/ml,TNF-αof(16.02±3.62)ng/L and CRP of(7.05±0.52)mg/L,which were lower than(14.02±2.63)pg/ml,(18.82±3.58)ng/L and(10.75±0.54)mg/L in the single group;the difference was statistically significant(P<0.05).Conclusion The treatment of omeprazole combined with colloidal bismuth pectin for peptic ulcer is beneficial to improve the inflammatory index of the patient,improve the treatment effect and clearance rate of Helicobacter pylori,reduce the recurrent rate and have a positive effect on promoting the prognosis of patients.
