摘　　要：目的探讨胃癌组织中凋亡抑制基因[基质金属蛋白酶-2(MMP-2)、Survivin]蛋白表达情况及其与幽门螺杆菌(Hp)感染的关系。方法以本院2020年5月—2022年5月收治的行胃癌根治性切除手术的90例胃癌患者作为研究对象,回顾性分析所选患者临床资料,收集其胃癌组织及癌旁组织。比较胃癌组织和癌旁组织survivin、MMP-2蛋白表达情况及Hp感染情况、胃癌组织survivin、MMP-2阳性表达情况及临床病理特征;胃癌患者组织survivin、MMP-2表达与Hp感染的相关性采用Pearman法分析;组织中survivin、MMP-2表达单独及联合检测对胃癌且Hp阳性的诊断价值采用受试者工作特征曲线(ROC)分析。结果胃癌组织中survivin、MMP-2阳性表达率高于癌旁组织(P<0.05);胃癌组织中Hp感染阳性多于癌旁组织的Hp感染阳性(P<0.05)。Survivin、MMP-2在胃癌组织Hp阳性中的表达阳性率明显高于Hp阴性(P<0.05)。胃癌患者TNM分期中,Ⅲ~Ⅳ的survivin、MMP-2阳性表达率较Ⅰ~Ⅱ明显增高(P<0.05);分化程度中,低分化的survivin、MMP-2阳性表达率较中高分化明显增高(P<0.05);浸润深度中,侵犯浆膜的survivin、MMP-2阳性表达率较未侵犯浆膜明显增高(P<0.05);有淋巴结转移的survivin、MMP-2阳性表达率较无淋巴结转移明显增高(P<0.05)。Pearman相关性分析结果显示,胃癌患者组织survivin、MMP-2表达与Hp感染呈正相关(r=0.427;0.484,P<0.05)。组织survivin、MMP-2表达单独及联合检测诊断胃癌的AUC分别为0.667、0.639、0.879。联合诊断的AUC高于各项单独检测,且联合检测的敏感度和特异度为84.40%,78.90%。结论胃癌患者组织Survivin、MMP-2呈高表达,且Survivin、MMP-2表达可能与肿瘤分期、分化程度、浸润深度及淋巴结转移密切相关;Hp感染与胃癌组织Survivin、MMP-2阳性表达情况呈正相关;联合检测组织Survivin、MMP-2表达有助于评估胃癌且Hp阳性患者病情。Objective To investigate the expression of apoptosis suppressor gene survivin and matrix metalloproteinase-2(MMP-2)protein in gastric cancer and their relationship with Helicobacter pylori(Hp)infection.Methods Ninety patients with gastric cancer who underwent radical gastrectomy in The Second People's Hospital of Wuxi from May 2020 to May 2022 were retrospectively selected as research objects.Clinical data of the selected patients were analyzed,and gastric cancer tissues and paracancer tissues were collected.The expression of survivin and MMP-2 in gastric and paracancer tissues and Hp infection,the positive expression of survivin and MMP-2 in gastric cancer tissues,and the clinicopathological features were compared.The correlation between the expressions of survivin and MMP-2 and Hp infection in gastric cancer patients was analyzed by Pearman method.Receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of survivin and MMP-2 in the diagnosis of Hp-positive gastric cancer.Results The survivin and MMP-2 positive expression rates in gastric cancer tissues were higher than those in paracancer tissues(P<0.05).The positive expression rates of Survivin and MMP-2 in Hp-positive gastric tissues were significantly higher than Hp-negative ones(P<0.05).In patients with gastric cancer,the positive expression rates of survivin and MMP-2 inⅢ~Ⅳwere significantly higher than those inⅠ~Ⅱ(P<0.05).In the degree of differentiation,survivin and MMP-2 positive expression rates of low-differentiation were significantly higher than those of medium-high differentiation(P<0.05).In the depth of infiltration,the survivin and MMP-2 positive expression rates of patients with serous membrane invasion were significantly higher than those without serous membrane invasion(P<0.05).The survivin and MMP-2 positive expression rates in patients with lymph node metastasis were significantly higher than those without lymph node metastasis(P<0.05).Pearman correlation analysis showed that survivin and MMP-2 expression
