HIF-1α、cyclin D1在膀胱癌组织中的表达及其临床病理意义

摘　　要：目的分析缺氧诱导因子1α(HIF-1α)、细胞周期素D1(cyclin D1)在膀胱癌组织中的表达及其临床病理意义。方法选取福建省龙岩市第一医院72例膀胱癌患者,均行手术治疗,取膀胱癌组织作为研究组,并取正常癌旁组织作为对照组,使用免疫组织化学法进行HIF-1α、cyclin D1的测定,分析HIF-1α阳性表达、cyclin D1阳性表达相关因素。结果研究组HIF-1α阳性率、cyclin D1阳性率显著高于对照组(P<0.05);不同临床分期、淋巴结转移情况、远处转移情况、病理分级患者HIF-1α阳性率、cyclin D1阳性率比较,差异有统计学意义(P<0.05);不同肿瘤直径患者HIF-1α阳性率比较,差异有统计学意义(P<0.05);经过Logistic回归分析发现,临床分期T3~T4、淋巴结转移、远处转移、病理分级G2~G3属于HIF-1α阳性表达、cyclin D1阳性表达危险因素(P<0.05),HIF-1α阳性表达危险因素还包括肿瘤直径>3 cm(P<0.05)。结论膀胱癌组织HIF-1α、cyclin D1呈过表达,均和临床分期、病理分化程度及转移密切相关。Objective To analyze the expression of hypoxia inducible factor 1α(HIF-1α)and cyclin D1 in bladder cancer tissues,and their clinicopathological significance.Methods A total of 72 patients with bladder cancer treated by surgery in Longyan First Hospital of Fujian Province were selected.Bladder cancer tissues were taken as the study group,and normal paracancerous tissues were taken as the control group.Immunohistochemical method was used to measure the levels of HIF-1αand cyclin D1.The factors related to positive expression of HIF-1αand cyclin D1 were analyzed.Results HIF-1αpositive rate and cyclin D1 positive rate in the study group were significantly higher than those in the control group(P<0.05).There were statistically significant differences in HIF-1αpositive rate and cyclin D1 positive rate in terms of different clinical stages,lymph node metastasis,distant metastasis,and pathological grades(P<0.05).HIF-1αpositive rates in patients with different tumor diameters were significantly different(P<0.05).Logistic regression analysis showed that clinical stage T3-T4,lymph node metastasis,distant metastasis and pathological grade G2-G3 were risk factors for positive expression of HIF-1αand cyclin D1(P<0.05).Tumor diameter larger than 3 cm was also a risk factor for positive expression of HIF-1α(P<0.05).Conclusions HIF-1αand cyclin D1 are overexpressed in bladder cancer tissues.Both are closely related to clinical stage,pathological differentiation and metastasis.
