不同HBV DNA载量慢性乙型肝炎患儿肝功能及凝血功能、血小板参数水平特征研究

摘　　要：目的研究不同乙型肝炎病毒脱氧核糖核酸(HBV DNA)载量慢性乙型肝炎患儿肝功能及凝血功能及血小板参数水平特征。方法选取2022年1月至2022年6月于开封市妇幼保健院诊治的149例慢性乙型肝炎患儿作为研究对象,根据HBV DNA载量不同分为低载量组(103拷贝/mL<HBV DNA载量≤105拷贝/mL)34例、中载量组(105拷贝/mL<HBV DNA载量≤107拷贝/mL)66例和高载量组(HBV DNA载量>107拷贝/mL)49例,抽取外周静脉血检测谷草转氨酶(ALT)、谷丙转氨酶(AST)、白蛋白(ALB)、总胆红素(TBIL)、活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)、凝血酶时间(TT)、纤维蛋白原(Fbg)、血小板计数(PLT)、平均血小板体积(MPV)、血小板分布宽度(PDW)和血小板压积(PCT)值。结果三组不同HBV DNA载量慢性乙型肝炎患儿的ALB、TBIL水平比较,差异无统计学意义(P>0.05),三组不同HBV DNA载量慢性乙型肝炎患儿的ALT、AST水平比较,差异有统计学意义(P<0.05),其中,ALT水平为低载量组<中载量组<高载量组,AST水平为低载量组<中载量组<高载量组。三组不同HBV DNA载量慢性乙型肝炎患儿的TT水平比较,差异无统计学意义(P>0.05),三组不同HBV DNA载量慢性乙型肝炎患儿的aPTT、PT及Fbg水平比较,差异有统计学意义(P<0.05),其中,aPTT水平为中载量组>高载量组,PT水平为低载量组<中载量组;中载量组>高载量组,Fbg水平为低载量组>高载量组。三组不同HBV DNA载量慢性乙型肝炎患儿的PDW和PCT水平比较,差异无统计学意义(P>0.05),三组不同HBV DNA载量慢性乙型肝炎患儿的PLT和MPV水平比较,差异有统计学意义(P<0.05),其中,PLT水平为低载量组>中载量组;低载量组>高载量组,MPV水平为低载量组<中载量组<高载量组。Spearman相关性分析结果显示,aPTT、PT水平与HBV-DNA载量之间,无明显相关性(P>0.05)、ALT、AST及MPV水平与HBV-DNA载量之间呈正相关,r分别为0.586、0.388和0.429、Fbg、PLT水平与HBVObjective To investigate the characteristics of liver function,coagulation function,and platelet parameters in children with chronic hepatitis B at different hepatitis B virus deoxyribonucleic acid(HBV DNA)levels.Methods A total of 149 children with chronic hepatitis B who were treated at Kaifeng Municipal Maternal and Child Health Hospital from January 2022 to June 2022 were selected as study subjects.They were divided into a low-load group(34 cases with 103 copies/mL<HBV DNA level≤105 copies/mL),a medium-load group(66 cases with 105 copies/mL<HBV DNA level≤107 copies/mL),and a high-load group(49 cases with HBV DNA level>107 copies/mL)based on their HBV DNA levels.Peripheral venous blood was drawn to test levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB),total bilirubin(TBIL),activated partial thromboplastin time(aPTT),prothrombin time(PT),thrombin time(TT),fibrinogen(Fbg),platelet count(PLT),mean platelet volume(MPV),platelet distribution width(PDW),and platelet count(PCT).Results There were no statistically significant differences in ALB and TBIL levels among the three groups of children with chronic hepatitis B at different HBV DNA loads(P>0.05).However,significant differences were found in ALT and AST levels among the three groups(P<0.05),with ALT levels being low-load group<medium-load group<high-load group,and AST levels being low-load group<medium-load group<high-load group.The TT levels among the three groups showed no significant difference(P>0.05).Significant differences were observed in aPTT,PT,and Fbg levels(P<0.05),with aPTT levels being medium-load group>high-load group,PT levels being low-load group<medium-load group;medium-load group>high-load group;and Fbg levels being low-load group>high-load group.There were no significant differences in PDW and PCT levels among the three groups(P>0.05).However,there were significant differences in PLT and MPV levels(P<0.05),where PLT levels were low-load group>medium-load group;low-load group>high-load group,and MPV leve
