摘 要:目的:基于核磁共振(MRI)测量前列腺周围脂肪厚度(PPFT),探讨其联合双参数MRI(Bp-MRI)诊断前列腺癌的效能及其对多西他赛化疗效果的价值。方法:选择2021年6月-2024年3月收治的有前列腺穿刺活组织检查指证并行前列腺穿刺活组织检查的232例患者的临床资料,按照病理结果分为前列腺癌组(n=64)和前列腺增生组(n=168),采用单因素及多因素Logistic回归筛选两组间的差异指标。分析PPFT对Bp-MRI诊断前列腺癌的辅助效能并绘制ROC曲线。此外,采用Pearson相关分析法描述PPFT与多西他赛化疗效果的相关性。结果:多因素Logistic回归筛选出PI-RADS评分和PPFT两个差异指标。前列腺癌组和前列腺增生组的PI-RADS评分与PPFT比较,曲线下面积(AUC)分别为0.841和0.679,敏感度分别为88%和59%,特异度分别为62%和72%,差异均有统计学意义(P<0.05)。PI-RADS评分联合PPFT诊断前列腺癌的AUC为0.873,敏感度69%,特异度92%,差异有统计学意义(P<0.05)。Pearson相关分析显示,PPFT的下降与多西他赛化疗后前列腺特异抗原(PSA)下降程度弱正相关(P=0.003),与高级别前列腺癌(GS≥8)呈弱正相关(P=0.015)。PPFT与体质量指数(BMI)、前列腺体积(PV)无相关性。结论:基于MRI测量的PPFT不仅能提升BP-MRI对前列腺癌的诊断效能,而且PPFT的下降与多西他赛化疗的敏感性有弱正相关性。Objective:To investigate the efficacy of periprostatic fat thickness(PPFT)based on magnetic resonance imaging(MRI)in assisting biparametric MRI(Bp-MRI)in the diagnosis of prostate cancer and its value of chemotherapy effect of docetaxel.Methods:The clinical data of patients with the indication of prostate biopsy who underwent prostate biopsy in hospital from June 2021 to March 2024 were collected,including PPFT measured by MRI,PI-RADSV2.1 score,pathological GleasonScore(GS)and clinical data of docetaxel before and after chemotherapy after prostate biopsy or radical prostatectomy.From June 2021 to March 2024,the clinical data of 232 patients with prostate biopsy were selected and divided into prostate cancer group(n=64)and prostate hyperplasia group(n=168)according to the pathological results.Single factor and multivariate Logistic regression were used to screen the difference indexes between the two groups.The diagnostic performance of PPFT with Bp-MRI was analyzed andthe ROC curve was drawn.Pearson correlation analysis was used to describe the correlation between PPFT and prostate cancer risk classification.Results:Multivariate Logistic regression screened out PI-RADS and PPFT.The area under the curve(AUC)of PI-RADS and PPFT in the diagnosis of prostate cancer was respectively 0.841 and 0.679,the sensitivity was 88%and 59%,the specificity was 62%and 72%(P<0.05).The AUC of PI-RADS combined with PPFT in the diagnosis of prostate cancer was 0.873,the sensitivity was 69%,and the specificity was 92%(P<0.05).Pearson correlation analysis showed that decrease of PPFT was positively correlated with the decrease of PSA after docetaxel chemotherapy(P=0.003)and high-grade prostate cancer(GS≥8)(P=0.015).There was no correlation between PPFT and body mass index(BMI)and prostate volume(PV).Conclusion:Periprostatic fat thicknessmeasured by MRI can not only improve the diagnostic efficiency of prostate cancer for biparametric MRI,but also have weak positive correlation with the sensitivity of docetaxel chemotherapy.
