摘 要:目的:分析唾液碳酸酐酶Ⅵ(CA-Ⅵ)及免疫生物标志物与低龄儿童龋(ECC)严重程度的相关性。方法:选取口腔科收治的ECC患儿146例(ECC组),根据龋病严重程度将其分为重度低龄儿童龋(SECC)组(n=79)和非SECC组(n=67),另选择100例口腔健康儿童为对照组。收集受检者清晨唾液标本,检测唾液中CA-Ⅵ、基质金属蛋白酶(MMP)-2、MMP-9、白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)、分泌性免疫球蛋白A(sIgA)及免疫球蛋白(Ig)A、IgG水平,并分析其与龋失补牙面指数(DMFS)的相关性及对SECC的诊断价值。结果:ECC组唾液CA-Ⅵ水平低于对照组(P<0.05),MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgG及IgA水平高于对照组(P<0.05);SECC组唾液CA-Ⅵ水平低于非SECC组(P<0.05),MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgG及IgA水平高于非SECC组(P<0.05)。相关性分析显示,ECC患儿唾液CA-Ⅵ与DMFS呈负相关(r=-0.516,P<0.05),唾液MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgA及IgG与DMFS呈正相关(r=0.558,0.474,0.420,0.375,0.434,0.336,0.373,P<0.05)。Possion回归分析显示,唾液CA-Ⅵ,MMP-2,MMP-9,IgA及IgG是DMFS的影响因素(P<0.05)。ROC曲线分析显示,CA-Ⅵ,MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgA及IgG鉴别SECC的曲线下面积分别为0.757,0.835,0.788,0.745,0.660,0.740,0.747及0.696。结论:ECC患儿唾液中CA-Ⅵ水平明显降低,免疫生物标志物水平明显升高,且与龋病严重程度密切相关。Objective:To analyze the correlation between carbonic anhydraseⅥ(CA-Ⅵ)and immune biomarkers in saliva and the severity of early childhood caries(ECC).Methods:One hundred and forty-six children with ECC(ECC group)were divided into severe early childhood caries(SECC)group(n=79)and non-SECC group(n=67)according to the severity of caries,and 100 healthy children were selected as the control group.The morning saliva samples of the subjects were collected,and the levels of CA-Ⅵ,matrix metalloproteinase(MMP)-2,MMP-9,interleukin(IL)-6,tumor necrosis factor-α(TNF-α),secretory immunoglobulin A(sIgA),immunoglobulin(Ig)A,and IgG in saliva were detected.The correlation between they and the decayed missing and filled surface(DMFS),and the diagnostic value of SECC were analyzed.Results:The level of CA-Ⅵin the saliva of the ECC group was lower than that of the control group(P<0.05).The levels of MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgG and IgA were higher than those in the control group(P<0.05).The level of salivary CA-Ⅵin the SECC group was lower than that of the non-SECC group(P<0.05).The levels of MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgA and IgG in the SECC group were higher than those in the non-SECC group(P<0.05).Correlation analysis showed that saliva CA-Ⅵwas negatively correlated with DMFS in children with ECC(r=-0.516,P<0.05).The levels of salivary MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgA and IgG were positively correlated with DMFS(r=0.558,0.474,0.420,0.375,0.434,0.336,0.373,P<0.05).Possion regression analysis showed that saliva CA-Ⅵ,MMP-2,MMP-9,IgA and IgG were the influencing factors of DMFS(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of identification SECC of CA-Ⅵ,MMP-2,MMP-9,IL-6,TNF-α,sIgA,IgA and IgG were 0.757,0.835,0.788,0.745,0.660,0.740,0.747 and 0.696,respectively.Conclusion:The level of CA-Ⅵin the saliva of children with ECC is significantly decreased,and the level of immune biomarkers is significantly increased,which is closely related to the severity of caries.
