加味大承气汤灌肠联合穴位敷贴在癌症晚期恶性肠梗阻治疗中的效果分析

摘　　要：目的 分析加味大承气汤灌肠联合穴位敷贴在癌症晚期恶性肠梗阻治疗中的效果。方法 选取2020年3月至2023年3月福建中医药大学附属漳州中医院收治的60例恶性肠梗阻患者,按照随机数表法将其分为观察组(30例)和对照组(30例)。对照组采用穴位敷贴治疗,观察组在对照组基础上加用大承气汤灌肠。比较两组患者的临床疗效、治疗后的肠功能康复相关指标、生活质量[卡氏功能状态评分(KPS)]及不良反应发生率。结果 观察组临床总有效率高于对照组(P <0.05);观察组治疗后肠鸣音恢复时间、腹痛腹胀缓解时间、排气恢复时间、排便恢复时间短于对照组(P <0.05);观察组治疗KPS评分高于对照组(P <0.05);观察组不良反应总发生率低于对照组(P <0.05)。结论 加味大承气汤灌肠联合穴位敷贴治疗可有效提高癌症晚期恶性肠梗阻临床效果,改善患者生活质量,降低不良反应发生率。Objective To analyze the effect of modified Dachengqi Decoction enema combined with acupoint application in the treatment of malignant intestinal obstruction in advanced cancer.Methods A total of 60 patients with malignant intestinal obstruction admitted to and treated in Zhangzhou Traditional Chinese Medicine Hospital Affiliated to Fujian University of Traditional Chinese Medicine from March 2020to March 2023 were selected and divided into the observation group(n=30) and the control group(n=30)according to the random number table method.The control group was treated with acupoint application,while the observation group was treated with modified Dachengqi Decoction enema on the basis of the control group.The clinical efficacy,postoperative indexes of intestinal functional rehabilitation,quality of life(Karnofsky performance status [KPS]) and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P < 0.05).After treatment,the recovery time of bowel sounds,the relief time of abdominal pain and distension,the recovery time of anal exhaust,and the recovery time of defecation in the observation group were shorter than those in the control group(P < 0.05).The KPS score of the observation group after treatment was higher than that of the control group(P < 0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group(P < 0.05).Conclusion Modified Dachengqi Decoction enema combined with acupoint application can effectively upgrade the clinical efficacy of malignant intestinal obstruction in advanced cancer,improve the QoL of patients and reduce the incidence of ADRs.
