摘　　要：目的:比较经皮微波消融(PMWA)、腹腔镜子宫肌瘤剔除术(LM)与开腹子宫肌瘤剔除术(AM)治疗子宫肌瘤患者的临床效果。方法:对2021年1月—2023年4月新钢中心医院92例子宫肌瘤患者临床资料进行回顾性分析,按手术方法将其分为PMWA组(31例)、LM组(28例)、AM组(33例)。比较三组的术中出血量、住院天数、术后肛门首次排气时间、并发症发生率、症状严重程度评分(SSS)及健康相关生命质量(HRQL)评分。结果:PMWA组的术中出血量和住院天数均少于LM组和AM组(P<0.05),术后肛门首次排气时间早于LM组和AM组(P<0.05)。三组并发症发生率比较,差异无统计学意义(P>0.05)。术后3、6、12个月,PMWA组的SSS均低于LM组和AM组(P<0.05),且HRQL评分均高于LM组和AM组(P<0.05)。结论:PMWA在治疗子宫肌瘤方面,相比LM和AM,具有术中出血量少、住院时间短、术后肠道恢复更快的优势,并能有效改善患者的临床症状和生活质量。Objective:To compare the clinical effects of percutaneous microwave ablation(PMWA),laparoscopic myomectomy(LM),and abdominal myomectomy(AM)in the treatment of patients with uterine fibroids.Method:The clinical data of 92 patients with uterine fibroids in Xingang Central Hospital from January 2021 to April 2023 were retrospectively analyzed,and they were divided into PMWA group(31 cases),LM group(28 cases)and AM group(33 cases)according to surgical methods.The amount of intraoperative blood loss,length of hospital stay,time of first anal exhaust after surgery,incidence of complications,symptom severity score(SSS)and health-related quality of life(HRQL)score were compared among the three groups.Result:The amount of intraoperative blood loss and hospitalization days in PMWA group were less than those in LM group and AM group(P<0.05),and the time of first anal exhaust after operation was earlier than that in LM group and AM group(P<0.05).There was no significant difference in the incidence of complications among the three groups(P>0.05).At 3,6 and 12 months after operation,SSS in PMWA group were lower than those in LM group and AM group(P<0.05),and HRQL scores were higher than those in LM group and AM group(P<0.05).Conclusion:In the treatment of uterine fibroids,compared with LM and AM,PMWA has the advantages of less intraoperative blood loss,shorter hospital stay,faster postoperative intestinal recovery,and can effectively improve the clinical symptoms and quality of life of patients.
