19例青少年川崎病的临床特征及转归分析

摘　　要：目的分析19例青少年川崎病(KD)的临床特征及转归。方法回顾性分析2021年1月至2024年7月于烟台毓璜顶医院儿内科住院的19例KD患儿的临床资料及诊治、随访情况。结果纳入的19例患儿中,18例为完全性KD。急性期,有2例冠状动脉病变,其中1例伴心功能下降;9例心肌酶或心电异常;14例伴不同其他系统表现。患儿发病至起始诊治时间为3~13 d,首剂静脉注射免疫球蛋白(IVIG)无应答率为78.9%,给予挽救治疗后效果良好。恢复期,1例出现左室扩大,其余均无并发症。结论青少年KD典型症状体征往往陆续出现,多同期存在其他系统症状,需仔细询问病史及查体以明确诊断。首剂IVIG应答效果多不佳,挽救治疗可获得较好的临床效果,降低冠状动脉病变的发生率。Objective To analyze the clinical features and prognosis in 19 cases of adolescents with Kawasaki disease(KD).Methods The clinical data,diagnosis,treatment and follow-up of 19 children with KD hospitalized in Department of Pediatric Internal Medicine,Yantai Yuhuangding Hospital from January 2021 to July 2024 were retrospectively analyzed.Results Of the 19 children included,18 had complete KD.In the acute stage,there were 2 cases of coronary artery disease,1 of whom was accompanied by decreased cardiac function.There were 9 cases of abnormal myocardial enzyme or electrocardiogram,14 cases had different manifestations of other systems.The time from onset to initial diagnosis and treatment was 3-13 days.The non-response rate of the first dose of intravenous immunoglobulin(IVIG)was 78.9%,and the effect was good after rescue treatment.In the convalescence period,left ventricular enlargement occurred in 1 case,and no complications occurred in the rest.Conclusion Typical symptoms and signs of KD in adolescents often appear one after another,and other systemic symptoms exist in most of the same period.Careful medical history and physical examination are needed to confirm the diagnosis.The response effect of the first dose of IVIG is not good,but salvage treatment can obtain better clinical effect and reduce the incidence of coronary artery disease.
