摘　　要：目的探讨成人癫痫患者使用抗癫痫药物后T淋巴细胞亚群变化情况及其影响作用。方法将110例癫痫患者作为研究对象,随机分为A组(n=57)和B组(n=53),选择同期体检的51例健康者作为对照组,A组给予卡马西平和丙戊酸镁进行治疗,B组单纯给予卡马西平进行治疗,对比观察研究组患者的治疗效果以及T淋巴细胞亚群变化情况。结果治疗前A组和B组患者IgA和IgG水平均明显低于健康对照组,治疗后2组研究组患者的IgA和IgG水平均明显升高,研究组改善程度优于对照组(P<0.05),研究组患者治疗前后IgM水平无明显变化,和对照组比较差异无统计学意义(P>0.05);治疗前A组和B组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均明显低于对照组,CD8^(+)高于对照组,治疗后研究组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均明显升高,CD8^(+)明显降低,A组患者改善程度优于B组(P<0.05);治疗前2组患者癫痫周发作频率比较差异无统计学意义(P>0.05),治疗后2组患者癫痫周发作频率明显降低,A组降低程度更为明显(P<0.05)。结论成人癫痫患者T淋巴细胞亚群水平明显异于健康人群,使用卡马西平和丙戊酸镁后能够改善患者IgA、IgG等免疫球蛋白水平和CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)等T淋巴细胞亚群水平,同时还会降低患者癫痫周发作频率,效果显著。Objective To investigate the changes of T lymphocyte subsets in adult patients with epilepsy and the effects of antiepileptic drugs.Methods A total of 110 patients with epilepsy who were randomly divided into study group A 57 cases and study group B 53 cases.51 healthy people who selected the same physical examination as a control group.Group A was treated with carbamazepine combined with Magnesium Valproate,group B was given carbamazepine for treatment,and the treatment effect of patients in the study group and the changes of T lymphocyte subsets were compared.Results The levels of IgA and IgG in group A and group B were significantly lower than those in healthy control group.The levels of IgA and IgG in the study group were significantly higher after treatment,and the improvement of the study group was better than that of the control group(P<0.05),there was no significant change in IgM levels before and after treatment in the study group.There was no significant difference between the two groups(P>0.05).Before treatment,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)were significantly lower than those in the control group,CD8^(+)was higher than the control group.After treatment,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)were significantly increased in the study group,CD8^(+)was significantly decreased,and the improvement in group A was better than that in group B(P<0.05).There was no significant difference in the frequency of weekly seizures(P>0.05).After treatment,the frequency of seizures was significantly lower in the two groups,and the degree of reduction in group A was more significant(P<0.05).Conclusion The levels of T lymphocyte subsets in adults with epilepsy are significantly different from those in healthy people.The use of carbamazepine combined with Magnesium Valproate can improve the levels of IgA,IgG and other immunoglobulins and CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),etc.The level of lymphocyte subsets also reduces the frequency of epileptic seizures in patients,and the effect is significant.
