盐酸曲唑酮联合右佐匹克隆对老年失眠伴焦虑抑郁患者睡眠质量及血清学指标的影响

摘　　要：目的探究盐酸曲唑酮联合右佐匹克隆对老年失眠伴焦虑抑郁患者睡眠质量及血清学指标的影响。方法选取2022年10月至2023年4月安阳市人民医院就诊的老年失眠伴焦虑抑郁患者100例,根据治疗方法不同分为对照组(n=50)、观察组(n=50)。其中对照组予以右佐匹克隆,观察组予以右佐匹克隆联合盐酸曲唑酮。比较两组临床疗效、不良反应发生率、治疗前后睡眠质量[匹兹堡睡眠质量指数(PSQI)评分]、血流动力学(血浆黏度、低切黏度、血小板聚集率、高切黏度)及血清学指标[神经肽Y(NPY)、脑源性神经生长因子(BDNF)、P物质(SP)]水平。结果观察组临床总有效率90.00%高于对照组74.00%(P<0.05)。治疗后,观察组血浆黏度、低切黏度、血小板聚集率、高切黏度低于对照组(P<0.05);观察组睡眠质量高于对照组(P<0.05);观察组NPY、BDNF、SP水平改善幅度大于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论右佐匹克隆联合盐酸曲唑酮治疗失眠伴焦虑抑郁疗效显著,可改善神经因子水平,提高睡眠质量,调节血流动力学,安全可靠。Objective To explore the effects of trazodone hydrochloride combined with dexzopiclone on sleep quality and serological indicators in elderly patients with insomnia accompanied by anxiety and depression.Methods A total of 100 elderly patients with insomnia accompanied by anxiety and depression who were treated in Anyang People's Hospital from October 2022 to April 2023,divided into a control group(n=50)and an observation group(n=50)based on different treatment methods.The control group was treated with dexzopiclone,and the observation group were treated with dexzopiclone combined with trazodone hydrochloride.The clinical efficacy and the incidence of adverse reactions were compared between the two groups,as well as the levels of sleep quality[Pittsburgh sleep quality index(PSQI)score],hemodynamics(plasma viscosity,low shear viscosity,platelet aggregation rate,high shear viscosity)and serological indexes[neuropeptide Y(NPY),brain-derived nerve growth factor(BDNF),Substance P(SP)]before and after treatment.Results The total effective rate of the observation group was 90.00%,which was higher than 74.00%of the control group(P<0.05).After treatment,the plasma viscosity,low shear viscosity,platelet aggregation rate and high shear viscosity in the observation group were lower than those in the control group(P<0.05);the sleep quality of the observation group was higher than that of the control group(P<0.05);the improvement levels of NPY,BDNF and SP in the observation group were greater than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Dexzopiclone combined with trazodone hydrochloride is effective in the treatment of insomnia with anxiety and depression,which can improve the levels of nerve factors,improve sleep quality,regulate hemodynamics,and is safe and reliable.
