摘  要:目的利用Meta分析方法系统评价miR-21的表达与头颈癌(HNC)诊断准确性的关联情况。方法数据库中检索2003年-2023年已发表的关于HNC中miR-21表达情况的临床研究文献,以及从基因表达综合数据库(GEO)收集HNC和健康组织样本miR-21的基因表达谱。根据相关纳入及排除标准进行文献筛选和质量评价,采用RevMan5.3和Stata17软件包对数据进行Meta分析。此外,本研究还预测了miR-21在HNC的靶基因并评价其生物学功能。结果共纳入文献8篇,其中HNC组784例,对照组476例。Meta分析结果显示:敏感性检测结果为Q=104.90(P<0.001),I2=86.65;异质性检验结果为Q=40.35(P<0.001),I2=65.3。数据结果显示研究对象的异质性较高,具有统计学意义,因此,选择随机效应模型,依次得出,诊断优势比(DOR)为20.96,灵敏度(SEN)为0.82,特异度(SPE)为0.83;正似然比(PLR)为4.69,负似然比(NLR)为0.22(全部P值<0.001)。SROC图显示4个研究对象分布在95%置信区间以内,曲线下方面积(AUC)为0.89。miR-21的表达量在HNC组和正常组差异有统计学意义。靶基因预测和功能分析筛选得到CASQ2、KLHL41、MYBPC1等8个核心靶标基因。结论HNC中miR-21的过表达可能通过抑制CASQ2、KLHL41、MYBPC1等靶标基因的作用来介导HNC的发生发展。临床可考虑将miR-21作为HNC诊断的潜在分子标志物。Objective To assess the relationship between the expression of miR-21 and its diagnostic accuracy of head and neck cancer(HNC)by meta-analysis.Methods All clinical articles concerning on the expression of miR-21 in HNC were electronically searched from the year of 2003 up to 2023.The miR-21 expression profile of HNC and matched healthy tissue samples was collected from gene expression omnibus(GEO).According to the inclusion and exclusion criteria,literature selection and quality evaluation were carried out,and consequently RevMan5.3 and Stata17 software packages were used for meta-analysis of diagnostic tests.In addition,this study also predicted the target genes of miR-21 in HNC and evaluated its biological function.Results A total of 8 articles were enrolled,including 784 cases in HNC group and 476 cases in control group.The differences were statistically significant in both sensitivity analysis and heterogeneity test[QS=104.90(P<0.001);Qh=40.35(P<0.001)].Accordingly,random effects model was performed for meta-analysis based on their high heterogeneous detection results(IS 2=86.65,Ih 2=65.3).Therefore,diagnostic odds ratio=20.96,sensitivity=0.82,specificity=0.83;positive likelihood ratio=4.69,negative likelihood ratio=0.22 all values SROC plot showed 4 subjects were distributed within 95%CI characterized by area under curve(AUC)=0.89 were presented statistically significant(all P<0.001).The expression of miR-21 in HNC group was significantly different from that in normal group.Eight hub target genes including CASQ2,KLHL41,and MYBPC1 were identified via target gene prediction and functional analysis.Conclusions MiR-21 may mediate the occurrence and development of HNC by inhibiting its target genes such as CASQ2,KLHL41,MYBPC1,etc.It can also be clinically considered as a potential molecular biomarker for the diagnosis of HNC.
