高强度聚焦超声消融术与腹腔镜子宫肌瘤剔除术治疗子宫肌瘤患者的效果比较

摘　　要：目的:比较高强度聚焦超声消融术与腹腔镜子宫肌瘤剔除术治疗子宫肌瘤患者的效果。方法:回顾性分析2021年1月至2023年6月该院收治的80例子宫肌瘤患者的临床资料,按照手术方法不同将其分成观察组与对照组各40例。对照组采用腹腔镜子宫肌瘤剔除术治疗,观察组采用高强度聚焦超声消融术治疗。比较两组手术前后性激素指标[促卵泡生成素(FSH)、黄体生成素(LH)、雌二醇(E2)]水平、子宫体积,以及复发率。结果:术后3个月,两组FSH、LH、E2水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);术后3个月,两组子宫体积均小于术前,且观察组小于对照组,差异有统计学意义(P<0.05);术后随访6个月,两组复发率比较,差异无统计学意义(P>0.05)。结论:高强度聚焦超声消融术治疗子宫肌瘤患者可降低性激素指标水平,以及缩小子宫体积的效果优于腹腔镜子宫肌瘤剔除术治疗。Objective:To compare effects of high intensity focused ultrasound ablation and laparoscopic myomectomy in treatment of patients with uterine fibroids.Methods:The clinical data of 80 patients with uterine fibroids admitted to this hospital from January 2021 to June 2023 were retrospectively analyzed.According to different surgical methods,they were divided into observation group and control group,40 cases in each group.The control group was treated with laparoscopic myomectomy,while the observation group was treated with high intensity focused ultrasound ablation.The levels of sex hormone indexes[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2)],the uterine volume before and after the surgery,and the recurrence rate were compared between the two groups.Results:Three months after the surgery,the levels of FSH,LH and E2 in the two groups were lower than those before the surgery,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Three months after the surgery,the uterine volumes of the two groups were smaller than those before the surgery,that of the observation group was smaller than that of the control group,and the differences were statistically significant(P<0.05).After 6 months of follow-up,there was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusions:High-intensity focused ultrasound ablation for the patients with uterine fibroids can reduce the levels of sex hormone indexes and reduce the uterine volume.Moreover,it is superior to laparoscopic myomectomy.
