超声引导下安珂真空辅助微创旋切术与环乳晕切口手术治疗多发性乳腺纤维腺瘤患者的效果比较

摘　　要：目的:比较超声引导下安珂真空辅助微创旋切术与环乳晕切口手术治疗多发性乳腺纤维腺瘤患者的效果。方法:选取2022年2月至2023年10月该院收治的80例多发性乳腺纤维腺瘤患者进行前瞻性研究,按照随机数字表法将其分为观察组与对照组各40例。对照组采用环乳晕切口手术治疗,观察组采用超声引导下安珂真空辅助微创旋切术治疗,比较两组手术相关指标水平(手术时间、切口愈合时间、术中出血量、术后1个月切口瘢痕长度、术后住院时间)水平、术后不同时间(术后6、12、24 h)视觉模拟评分法(VAS)评分、术后1个月乳房表面美观评分(BSBs)和术后3个月并发症发生率。结果:观察组手术时间、切口愈合时间、术后1个月切口瘢痕长度及术后住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后6、12、24 h,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);术后1个月,观察组乳头乳晕感觉、乳房形状、乳房切口瘢痕、色素沉着等BSBs评分均高于对照组,差异有统计学意义(P<0.05);术后3个月,观察组并发症发生率为5.00%,明显低于对照组的25.00%,差异有统计学意义(P<0.05)。结论:超声引导下安珂真空辅助微创旋切术治疗多发性乳腺纤维腺瘤患者可提高术后BSBs评分,缩短手术时间、切口愈合时间、术后1个月切口瘢痕长度及术后住院时间,减少术中出血量,降低VAS评分和并发症发生率,效果优于环乳晕切口手术治疗。Objective:To compare effects of ultrasound-guided Anke vacuum-assisted minimally invasive circumcision and circumferential areola incision in treatment of patients with multiple breast fibroadenoma.Methods:A prospective study was conducted on 80 patients with multiple breast fibroadenoma admitted to the hospital from February 2022 to October 2023.According to the random number table method,they were divided into observation group and control group,40 cases in each group.The control group was treated with areola incision,while the observation group was treated with ultrasound-guided Anke vacuum-assisted minimally invasive circumcision.The levels of operationrelated indexes(operation time,incision healing time,intraoperative blood loss,incision scar length 1 month after the surgery,postoperative hospitalization time),the visual analogue scale(VAS)score at different time after the surgery(6,12,24 h after the surgery),the breast surface aesthetics score(BSBs)1 month after the surgery,and the incidence of complications 3 months after the surgery were compared between the two groups.Results:The operation time,the incision healing time,the incision scar length 1 month after the surgery,and the postoperative hospitalization time in the observation group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).At 6,12,and 24 h after the surgery,the VAS scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).At 1 month after the surgery,the BSBs scores of nipple areola sensation,breast shape,breast incision scar,and pigmentation in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,3 months after the surgery,the incidence of complications in the observation group was 5.00%,which was significantly lower than 25.00%of the control group,and the differenc
