摘 要:目的分析颈动脉超声评估静息状态及Valsalva动作下卵圆孔未闭与脑梗死灶分布、部位和数量的关系。方法选取2021年1月至2023年12月在本院就诊的脑卒中患者为研究对象,共计137例,根据患者卵圆孔未闭(Patent foramen ovale,PFO)的情况,将脑梗死患者分为PFO组(n=62)和无PFO组(n=75),同期选取体检科健康受试者设为对照组(n=70),分析静息状态下和Valsalva动作下脑梗死患者与健康对照组右向左分流情况,比较PFO以及非PFO患者脑梗死灶分布、部位和数量情况。结果在静息状态下和Valsalva动作下脑梗死患者与健康对照组右向左分流程度有显著差异,同时在Valsalva动作下,脑梗死患者的右向左分流程度比静息状态下显著增加,PFO组患者与非PFO患者的脑梗死灶分布、部位和数量情况均有显著性差异,PFO组患者在后循环供血区的情况较多,脑梗灶数量较多以多发为主(P<0.05)。结论PFO与脑梗死灶分布、部位和数量之间存在一定的关系,PFO相关脑梗死灶分布更广泛,部位更倾向于发生在后循环,数量更多,这可能右向左分流导致的多发性微栓子有关,为PFO相关脑梗死的预防和治疗提供了依。Objective Analyze the relationship between the distribution,location,and quantity of cerebral infarction lesions and patent foramen ovale under resting state and Valsalva maneuver evaluated by carotid artery ultrasound.Methods A total of 137 stroke patients who visited our hospital from January 2021 to December 2023 were selected as the research subjects.Based on the condition of patent foramen ovale(PFO),stroke patients were divided into PFO group(n=62)and no PFO group(n=75).During the same period,healthy subjects from the physical examination department were selected as the control group(n=70),and the right to left shunt between stroke patients and the healthy control group was analyzed under resting state and Valsalva action,Compare the distribution,location,and quantity of cerebral infarction foci between PFO and non PFO patients.Results There is a significant difference in the degree of right to left shunting between patients with cerebral infarction and the healthy control group under resting state and Valsalva action.At the same time,under Valsalva action,the degree of right to left shunting in patients with cerebral infarction is significantly increased compared to resting state.There are significant differences in the distribution,location,and quantity of cerebral infarction lesions between PFO group patients and non PFO patients.PFO group patients have more posterior circulation supply areas,and the number of cerebral infarction lesions is mainly multiple(P<0.05).Conclusion There is a certain relationship between PFO and the distribution,location,and quantity of cerebral infarction foci.PFO related cerebral infarction foci are more widely distributed and tend to occur in the posterior circulation,with a greater number.This may be related to multiple microemboli caused by right to left shunting,providing a basis for the prevention and treatment of PFO related cerebral infarction.
