摘　　要：目的检测Trx-1及HIF-1α在胃癌腹膜转移者、未转移者中的表达情况,分析其在胃癌腹膜转移中的临床意义。方法免疫组化法检测Trx-1及HIF-1α在胃癌腹膜转移患者原发灶、未转移患者原发灶中的表达情况;利用相关分析分析Trx-1及HIF-1α的相关关系;采用Kaplan-Meier法分析Trx-1及HIF-1α阳性和HIF-1α阴性表达者的平均生存时间;Logistic回归分析胃癌腹膜转移的危险因素。结果Trx-1及HIF-1α在胃癌腹膜转移组与胃癌腹膜未转移组中的表达差异有统计学意义(P<0.05)。相关分析提示,Trx-1与HIF-1α表达呈正相关。Trx-1及HIF-1α在胃癌腹膜转移灶中的阳性表达与肿瘤的Borrmann分型及T分期相关。Trx-1及HIF-1α表达阳性的患者术后生存时间短于Trx-1及HIF-1α表达阴性的患者。Logistic回归分析提示,原发肿瘤Borrmann分型、T分期,以及原发灶中Trx-1及HIF-1α的阳性表达是胃癌腹膜转移的危险因素。结论Trx-1及HIF-1α的表达与胃癌腹膜转移有关,且高表达者预后不良。Objective To investigate the expression of Trx-1 and HIF-1αin the gastric carcinoma with or without the peritoneal metastases,analyze its role in the peritoneal metastasis of gastric cancer.Methods The Trx-1 and HIF-1αexpression were detected by immunohistochemistry method.The correlation of Trx-1 and HIF-1αwas used with correlation analysis.Kaplan-Meier test was used to analyze the mean survival time of the patients with expression of positive and negative Trx-1 and HIF-1α.Logistic regression analysis was used for evaluating the risk factors of peritoneal metastasis of gastric cancer.Results Trx-1 and HIF-1αexpression had significant differences between those patients with peritoneal metastasis and those without.Correlation analysis suggests Trx-1 expression was positively correlated with HIF-1α.Trx-1 and HIF-1αpositive expression in the peritoneal metastases correlated with Borrmann type and depth of tumor invasion.Those patients with Trx-1 and HIF-1αpositive expression had shorter survival time than the negative ones.Primary tumor Borrmann type,depth of tumor invasion,Trx-1 and HIF-1αpositive expression were risk factors of tumor peritoneal metastasis of gastric cancer.Conclusion The high expression of Trx-1 and HIF-1αis associated with gastric cancer peritoneal metastasis and poor prognosis.
