血清ProGRP、SCCAg、Cyfra21-1和CEA检测对肺癌的诊断价值研究

摘  要:目的:研究血清胃泌素释放肽前体(ProGRP)、鳞状上皮细胞癌抗原(SCCAg)、细胞角蛋白19片段抗原(Cyfra21-1)和癌胚抗原(CEA)检测对肺癌的诊断价值。方法:选取2022年5月—2023年5月开封市第三人民医院收治的74例肺癌患者作为肺癌组,同时选择同期收治的80例肺部良性病变患者作为良性组。采集两组患者空腹静脉血,采用电化学发光法测定ProGRP、Cyfra21-1水平,采用酶联免疫吸附法测定SCCAg、CEA水平。比较两组患者ProGRP、SCCAg、Cyfra21-1和CEA水平,以病理诊断结果作为金标准,计算ProGRP、SCCAg、Cyfra21-1和CEA单项与联合诊断肺癌敏感度、特异度、准确率及与病理结果的一致性。结果:肺癌组ProGRP、SCCAg、Cyfra21-1和CEA水平高于良性组,差异均有统计学意义(t=30.576、14.998、15.917、17.420,P<0.05);ProGRP+SCCAg+Cyfra21-1+CEA联合诊断肺癌敏感度、准确率高于ProGRP、SCCAg、Cyfra21-1和CEA单项诊断,差异有统计学意义(P<0.05)。结论:肺癌患者血清ProGRP、SC-CAg、Cyfra21-1和CEA水平升高,血清ProGRP、SCCAg、Cyfra21-1和CEA联合检测能够更为灵敏、准确检出肺癌,为肺癌的诊断提供更为全面及准确的依据。Objective:To investigate the diagnostic value of serum gastrin releasing peptide precursor(ProGRP),squamous cell carcinoma antigen(SCCAg),cytokeratin 19 fragment antigen(Cyfra21-1),and carcinoembryonic antigen(CEA)detection in lung cancer.Methods:74 lung cancer patients who were treated in the hospital from May 2022 to May 2023 were selected as the lung cancer group,while 80 patients with benign lung lesions who were treated in the hospital during the same period were selected as the benign group.Two groups of fasting venous blood were collected,and the levels of ProGRP and Cyfra21-1 were measured using electrochemical luminescence method.The levels of SCCAg and CEA were measured using enzyme-linked immunosorbent assay.The levels of ProGRP,SCCAg,Cyfra21-1,and CEA of two groups were compared.Pathological diagnosis results were used as the gold standard to calculate the sensitivity,specificity,accuracy,and consistency with pathological results of ProGRP,SCCAg,Cyfra21-1 and CEA for single and combined diagnosis of lung cancer.Results:The levels of ProGRP,SCCAg,Cyfra21-1 and CEA in the lung cancer group were higher than those in the benign group,with statistically significant difference(t=30.576,14.998,15.917,17.420;P<0.05).The sensitivity and accuracy of the combined diagnosis of lung cancer with ProGRP+SCCAg+Cyfra21-1+CEA were higher than those of ProGRP,SCCAg,Cyfra21-1 and CEA,with statistically significant differnce(P<0.05).Conclusion:The serum levels of ProGRP,SCCAg,Cyfra21-1 and CEA in lung cancer patients increase.The combined detection of ProGRP,SCCAg,Cyfra21-1,and CEA in serum can more sensitively and accurately detect lung cancer,providing a more comprehensive and accurate basis for the diagnosis of lung cancer.
