摘 要:目的探讨微螺钉种植体不同植入方法对口腔菌群、系统稳定性的影响。方法选取2019年6月至2021年6月于萍乡市人民医院进行口腔正畸的88例患者(168枚种植体)作为研究对象,根据微螺钉种植体植入方法不同分为自攻型组与助攻型组,每组44例(84枚种植体)。自攻型组采用自攻型植入微螺钉种植体,助攻型组采用助攻型植入微螺钉种植体。比较两组种植成功率、临床疗效、种植体周围软组织相关指标[改良龈沟出血指数(modified sulcus bleeding index,mSBI)、牙周探诊深度(periodontal depth,PD)]、口腔菌群数量及骨吸收值。结果自攻型组种植体存留率为97.62%,高于助攻型组的89.29%,差异有统计学意义(P<0.05)。自攻型组治疗总有效率为93.18%,高于助攻型组的65.91%,差异有统计学意义(P<0.05)。植入后1、3、6个月,自攻型组mSBI均低于助攻型组,PD均浅于助攻型组,差异有统计学意义(P<0.05)。植入后3、6个月,自攻型组放线菌数量少于助攻型组,差异有统计学意义(P<0.05)。植入后3、6个月,自攻型组骨吸收值均低于助攻型组,差异有统计学意义(P<0.05)。结论口腔正畸患者采用自攻型微螺钉种植体矫治效果显著,种植体稳定性好,且患者种植体周围软组织健康状况佳,造成的口腔炎症轻微,可减轻牙槽骨的吸收,值得临床推广应用。Objective To compare the effects of different implantation methods of microscrew implants on oral flora and system stability.Methods A total of 88 patients(168 implants)who underwent orthodontics in Pingxiang People's Hospital from June 2019 to June 2021 were selected as the research subjects,and they were divided into the self-attacking group and the auxiliary group according to the different implantation methods of microscrew implants,with 44 cases(84 implants)in each group.The self-attacking group was implanted with self-attacking microscrew implant,and the auxiliary group was implanted with auxiliary microscrew implant,the implantation success rate and clinical efficacy,and the peri-implant soft tissue related indexes(modified sulcus bleeding index[mSBI],periodontal depth[PD]),oral flora number and bone resorption value were compared between the two groups.Results The retention rate of implant in the self-attacking group was 97.62%,which was higher than 89.29%in the auxiliary group,and the difference was statistically significant(P<0.05).The total clinical effective rate in the self-attacking group was 93.18%,which was higher than 65.91%in the auxiliary group,and the difference was statistically significant(P<0.05).At 1,3 and 6 months after implantation,mSBI in the self-attacking group was lower than that in the auxiliary group,and PD was lower than that in the auxiliary group,and the difference was statistically significant(P<0.05).At 3 and 6 months after implantation,the number of actinomyces in the self-attacking group was lower than that in the auxiliary group,and the difference was statistically significant(P<0.05).At 3 and 6 months after implantation,the bone resorption value in the self-attacking group was lower than that in the auxiliary group,and the difference was statistically significant(P<0.05).Conclusion Self-attacking microscrew implants for orthodontic patients have remarkable effects,good implant stability,and good soft tissue health around the implants,resulting in slight oral inflammation,w
