三级医院医师医学科研伦理知信行素养现状分析

摘　　要：目的了解三级医院医师医学科研伦理知信行现状,为增强其伦理意识、促进医学研究的规范化开展提供参考。方法采用文献查阅、专家咨询等方法确定调查表,通过河南省医师协会40个专科分会向河南省三级医院医师发放调查表2067份。对医师医学科研伦理认知与伦理行为进行分析。结果回收有效问卷1694份,有效回收率为81.95%;医师对医学伦理尊重(自主)原则、不伤害原则、有利(行善)原则、公正原则知晓率均>80.00%,4项原则全部知晓1153人,知晓率为68.06%;仅12.04%的医师对伦理委员会审查程序非常了解。97.46%的医师认为医学技术的发展需要伦理的规范和支持,95.11%的医师认为科研伦理认知的提高与医学科研质量的提高相关。1358名开展过医学研究的医师中,87.03%医师在进行医学科研时申请了伦理委员会审查。Pearson相关性分析结果显示,医师医学科研伦理认知与伦理行为呈正向相关(r>0,P<0.05)。结论医师对医学伦理基本原则知晓率较高,但对实践层面伦理审查程序知晓水平较低;科研活动中加强伦理审查管理很有必要;对医师科研伦理知信行持续深入研究以应对科技发展中不断涌现的伦理问题。Objective To understand the current situation of doctors'knowledge,attitude and practice of physicians in tertiary hospitals,and to provide reference for enhancing their ethical awareness and promoting the standardization of medical research.Methods The questionnaire was de‐termined by literature review,expert consultation and other methods,a total of 2067 questionnaires were distributed to physicians in tertiary hospi‐tals in Henan Province through 40 specialized branches of Henan Medical Association.To analyze the ethical cognition and ethical behavior of physi‐cians in medical scientific research.Results 1694 valid questionnaires were collected,with an effective recovery rate of 81.95%;the awareness rate of principles of respect(autonomy),non-harm,benefit(doing good)and justice of medical ethics were all>80.00%,and 1153 people were aware of all the four principles,with an awareness rate of 68.06%;only 12.04%of the physicians were very familiar with the review procedure of the ethics committee.97.46%of the physicians believed that the development of medical technology needed ethical norms and support,and 95.11%of the phy‐sicians believed that the improvement of scientific research ethics cognition was related to the improvement of medical research quality.Of the 1358 physicians who had conducted medical research,87.03%applied for the review of the ethics committee during medical research.Pearson correlation analysis showed that there was a positive correlation between physicians'ethical cognition and ethical behavior in medical research(r>0,P<0.05).Conclusion Physicians have a high awareness of the basic principles of medical ethics,but a low level of awareness of the ethical review process at the practical level;it is necessary to strengthen the management of ethical review in scientific research activities;continue to conduct in-depth research on physicians'scientific research ethics to cope with the emerging ethical issues in the development of science and technology.
