摘 要:目的探讨MRI联合CT定位对头颈部肿瘤的放疗靶区勾画的影响。方法选取2020年7—12月在江油市第二人民医院进行放疗的50例头颈部肿瘤患者作为研究对象,根据检查方法不同分为对照组与研究组,每组25例。对照组通过CT定位进行放疗靶区勾画,研究组通过MRI联合CT定位进行放疗靶区勾画。比较两组勾画的大体肿瘤靶区(gross tumor volume,GTV)相关参数(长度、厚度和体积)、GTV相关参数(C.V值、Ratio值)、放疗后疗效及放疗相关不良反应发生情况。结果研究组GTV长度短于对照组,GTV厚度薄于对照组,GTV体积小于对照组,差异有统计学意义(P<0.05)。研究组GTV长度、厚度、体积CV值均小于对照组,差异有统计学意义(P<0.05);两组GTV长度、厚度、体积Ratio值比较差异无统计学意义。两组完全缓解率、部分缓解率、稳定率和进展率比较差异均无统计学意义。研究组放疗相关不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论相比于传统单纯CT定位下靶区勾画,MRI联合CT定位下勾画头颈部肿瘤放疗靶区能提高其勾画的精准性,减少对正常组织的放疗损伤,减少放疗不良反应的发生。Objective To investigate the effect of MRI combined with CT localization on the delineation of radiotherapy target area of head and neck tumors.Methods 50 patients with head and neck tumors who underwent radiotherapy in the Jiangyou Second People's Hospital from July to December 2020 were selected as the research subjects,they were divided into the control group and the study group according to different examination methods,with 25 cases in each group.The radiotherapy target area was delineated by CT positioning in the control group,and the radiotherapy target area was delineated by MRI combined with CT positioning in the study group.The gross tumor volume(GTV)related parameters(length,thickness and volume),GTV related parameters(C.V value,Ratio value),efficacy after radiotherapy and radiotherapy-related adverse reactions were compared between the two groups.Results The length of GTV in the study group was shorter than that in the control group,the thickness of GTV was thinner than that in the control group,and the volume of GTV was smaller than that in the control group,the differences were statistically significant(P<0.05).The CV values of GTV length,thickness and volume in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05);there was no significant difference in GTV length,thickness and volume Ratio between the two groups.There was no significant difference in complete remission rate,partial remission rate,stability rate and progression rate between the two groups.The incidence of radiotherapy-related adverse reactions in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Compared with the traditional target delineation under CT localization,the delineation of the radiotherapy target area of head and neck tumors under MRI combined with CT localization can improve the accuracy of the delineation,reduce the radiotherapy damage to normal tissues,and reduce the incidence of radiot
