蜂针在面瘫急性期患者中的应用效果及对面动脉血流动力学的影响

摘　　要：目的探讨蜂针对面瘫急性期患者面动脉血流动力学的影响。方法选取2022年9月至2024年4月昆明市中医医院收治的72例面瘫急性期患者作为研究对象,按照随机数字表法分为观察组(36例)和对照组(36例)。观察组采用蜂针治疗方法,对照组采用普通针刺治疗方法。比较两组的临床疗效、血液流变学指标、凝血功能、面动脉最大直径及患侧面动脉收缩期峰值流速。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的全血黏度1.00、全血黏度5.00、全血黏度200.00、血浆黏度均低于对照组,差异有统计学意义(P<0.05),两组的全血黏度10.00比较,差异无统计学意义(P>0.05)。观察组治疗后的凝血酶原时间、活化部分凝血酶时间、凝血酶时间均长于对照组,纤维蛋白原低于对照组,差异有统计学意义(P<0.05)。观察组治疗后的面动脉最大直径宽于对照组,面动脉收缩期峰值流速低于对照组,差异有统计学意义(P<0.05)。结论蜂针可能通过降低血液阻力,扩张血管,改善血流灌注量起到治疗作用,对早期面瘫患者的介入具有积极意义,值得推广。Objective To investigate the effect of bee acupuncture on the hemodynamics of the facial artery in patients with acute facial paralysis.Methods A total of 72 patients with acute stage facial palsy admitted to Kunming Hospital of Traditional Chinese Medicine from September 2022 to April 2024 were selected as the study objects and they were divided into observation group(36 cases)and control group(36 cases)according to random number table method.The observation group was treated with bee acupuncture,while the control group received regular acupuncture.The clinical efficacy,hemorheology index,coagulation function,maximum diameter of facial artery and peak systolic flow rate of affected lateral artery were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,whole blood viscosity of 1.00,whole blood viscosity of 5.00,whole blood viscosity of 200.00 and plasma viscosity of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05),while the whole blood viscosity of 10.00 between the two groups was not statistically significant(P>0.05).After treatment,the prothrombin,activated partial thrombin time and thrombin time in observation group were longer than those in control group,but the fibrinogen was lower than that in control group,the differences were statistically significant(P<0.05).After treatment,the maximum diameter of facial artery in observation group was wider than that in control group,and the peak velocity of facial artery systolic period was lower than that in control group,the differences were statistically significant(P<0.05).Conclusion Bee acupuncture may play a therapeutic role by reducing blood resistance,dilating blood vessels,and improving blood perfusion.It has positive significance for early intervention in patients with facial paralysis and is worth promoting.
