颈部血管超声用于缺血性脑血管病的临床研究

摘　　要：目的评估颈部血管超声检查在缺血性脑血管病诊断中的应用价值。方法收集2021年6月至2024年6月收治的缺血性脑血管病患者作为观察组,另将同期体检的健康者120例作为对照组。比较2组颈部血管超声颈动脉斑块检测结果、颈动脉狭窄程度、颈动脉内径及颈动脉内-中膜厚度以及血流动力学参数(脑血流量、舒张期最小及最大血流速度)。结果观察组颈动脉斑块检出率为95.00%大于对照组的2.50%(P<0.01)。观察组颈动脉狭窄检出率为93.33%高于对照组的1.67%(P<0.01)。观察组颈动脉内径小于对照组,颈动脉内-中膜厚度大于对照组(P<0.01)。观察组脑血流量、颈动脉舒张期最小血流速度及舒张期最大血流速度均低于对照组(P<0.01)。结论颈部血管超声检查可以随时获得颈动脉血流动力学参数,精确测定每一段血管腔的狭窄状态、斑块位置及血管内-中膜厚度,可为体检防病、入院治病提供准确可靠的诊断依据。Objective To evaluate the value of cervical vascular ultrasonography in the diagnosis of ischemic cerebro⁃vascular disease.Methods Patients with ischemic cerebrovascular disease admitted from June 2021 to June 2024 were se⁃lected as the observation group,and 120 healthy subjects who underwent physical examination during the same period were se⁃lected as the control group.The results of carotid plaque detection,carotid stenosis degree,carotid inner diameter,carotid in⁃tima⁃media thickness and hemodynamic parameters(cerebral blood flow,minimum and maximum diastolic blood flow veloci⁃ty)were compared between the two groups.Results The detection rate of carotid plaque in observation group was 95.00%,which was higher than that in control group 2.50%(P<0.01).The detection rate of carotid artery stenosis was 93.33%in the observation group and 1.67%in the control group(P<0.01).The carotid artery diameter in the observation group was smaller than that in the control group,and the carotid intima⁃media thickness was higher than that in the control group(P<0.01).The cerebral blood flow,minimum diastolic blood flow velocity and maximum diastolic blood flow velocity in the obser⁃vation group were lower than those in the control group(P<0.01).Conclusion Ultrasound detection of cervical blood ves⁃sels can obtain the parameters of carotid blood flow dynamics at any time,accurately determine the stenosis state,plaque loca⁃tion and endovascular⁃media thickness of each segment of vascular lumen,and provide accurate and reliable diagnostic basis for disease prevention and hospitalization.
