郑州地区表观健康人群血清脂联素参考区间的建立与验证

摘　　要：目的检测郑州地区表观健康人群的血清脂联素(serum adiponectin,ADPN)水平,进行性别和年龄差异分析并建立和验证适合该地区人群的血清脂联素参考区间。方法依据美国临床实验室标准化协会(CLSI)C28-A3指南,选取河南省人民医院健康管理中心2023年1月至2023年5月表观健康人群作为研究对象,排除患代谢性疾病以及脂代谢异常人群,共收集948份标本。其中男性受试者430名,女性受试者518名,分别按年龄段18~30岁,31~40岁,41~50岁,51~60岁,>60岁分成五组。采用胶乳增强免疫比浊法检测其血清脂联素水平,以百分位数法建立血清脂联素的参考区间。另每个年龄段收集20例体检健康者血清样本,共计100例样本,进行参考区间验证,当比率值R大于等于90%,判断验证通过。结果郑州地区健康成年人的脂联素水平呈偏态分布,男性血清脂联素中位值为5.84 mg/L,女性血清脂联素中位值为5.83 mg/L,两组间差异无统计学意义(Z=1.261,P=0.207);五个年龄组间差异有统计学意义(H=32.060,P<0.001),成对比较显示,≤50岁的三个年龄组间和>50岁的两个年龄组间差异无统计学意义(均P>0.05),>50岁年龄组血清脂联素水平显著高于≤50岁年龄组(Z=4.757,P=0.000)。将脂联素>P25范围定义为参考值。脂联素总体参考区间为:>4.71 mg/L;≤50岁脂联素参考区间为:>4.66 mmg/L;>50岁脂联素参考区间为:>5.03 mg/L。100例体检健康者的比率值R为0.93,新建立的参考区间验证通过。多因素线性回归分析显示,血清脂联素水平受年龄、空腹血糖、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇和载脂蛋白E影响(标准化回归系数β=0.024、-0.693、2.193、-0.851、-0.399,0.297,均P<0.05)。结论本研究初步建立了郑州地区表观健康人群血清脂联素的参考区间,为糖尿病、代谢综合征以及心血管疾病的早期筛查提供了参考标准。Objective To detect serum adiponectin(ADPN)levels in healthy people in Zhengzhou,and then analyze the gender and age differences and establish the reference interval for adiponectin of the population in this area.Methods According to the C28-A3 guideline of the American Clinical Laboratory Standardization Association(CLSI),the healthy physical examination population from the health management center of Henan Provincial People's Hospital from January 2023 to May 2023 were selected as the research subjects.After excluding people with metabolic diseases and abnormal lipid metabolism,a total of 948 compliantspecimens were collected.There were 430 male subjects and 518 female subjects,and the age was divided into five groups:18-30,331-40,41-50,51-60 and>60 years old.Serum adiponectin levels were measured by latex enhanced immunoturbidimetry,and the reference interval of serum adiponectin was established by percentile method according to gender and age.Results Adiponectin levels showed a skewed distribution,The median value of serum adiponectin was 5.84 mg/L in male and 5.83 mg/L in female,the adiponectin levels did not differ between genders(Z=1.261,P=0.207);There were significant differences among the 5 age groups(H=32.060,P<0.001),paired comparison showed that there was no significant difference among three age groups≤50 years old and between the two age groups of>50 years old(all P>0.05).Therefore,And people over the age of 50 serum adiponectin levels are significantly higher than people under the age of 50(Z=4.757,P<0.001).The range of adiponectin>P25 was defined as the reference value.The overall adiponectin reference interval was>4.71mg/L;≤50 years old adiponectin reference interval was 4.66 mg/L;>50 years old adiponectin reference interval:>5.03mg/L.The R value of 100 cases of healthy adults physical examination was 0.93,the newly established reference interval of serum adiponectin passed the verification.Multi-factor linear regression analysis showed that the serum adiponectin level was affected by age,fas
