心理弹性在宫颈癌患者希望水平与癌因性疲乏间的中介效应

摘　　要：目的调查宫颈癌患者癌因性疲乏、希望水平及心理弹性现状,并探讨心理弹性在宫颈癌患者希望水平与癌因性疲乏间的中介效应。方法采用便利抽样法,选取2022年1月-2023年1月我院收治的宫颈癌患者268例为研究对象,采用一般资料调查表、心理弹性量表(CD-RISC)、中文版Herth希望量表(HHI)、癌因性疲乏量表(CFS)进行调查,构建结构方程模型(SEM)并进行中介效应检验。结果268例宫颈癌患者CD-RISC得分为(48.83±5.12)分、HHI得分为(34.65±4.31)分、CFS得分为(31.27±4.06)分;宫颈癌患者癌因性疲乏与希望水平、心理弹性均呈负相关(r=-0.584、-0.736,P<0.05);希望水平与心理弹性呈正相关(r=0.774,P<0.05);心理弹性在希望水平与癌因性疲乏间的间接效应值为0.20,占总效应的27.03%。结论宫颈癌患者心理弹性、希望水平、癌因性疲乏均处于中等水平,希望水平可通过心理弹性间接预测癌因性疲乏,具有中介效应,护理人员应加强对患者心理弹性的关注与干预,提高其希望水平,减轻癌因性疲乏。Objective To investigate the status quo of cancer-induced fatigue,hope level and mental resilience in patients with cervical cancer,and to explore the mediating role of mental resilience on the cancer-induced fatigue and hope level.Methods 268 cases of cervical cancer patients admitted to Shanxi Bethune Hospital from January 2022 to January 2023 were selected as the research objects.General data questionnaire,mental resilience scale(CD-RISC),Chinese version of Herth hope scale(HHI)and cancer fatigue scale(CFS)were used to conduct questionnaire investigation.Structural equation model(SEM)was established and mediating effect test was conducted.Results The scores of CD-RISC、HHI、CFS were(48.83±5.12),(34.65±4.31)and(31.27±4.06)in 268 patients with cervical cancer.In cervical cancer patients,cancer-induced fatigue was negatively correlated with hope level and mental resilience(r=-0.584、-0.736,P<0.05).There was a positive correlation between hope level and mental resilience(r=0.774,P<0.05).The mediating effect of mental resilience between self-hope level and cancer-induced fatigue was 0.20,accounting for 27.03%of the total effect.Conclusion The scores of mental resilience,hope level and cancer-related fatigue in patients with cervical cancer were all at medium level.Hope level can indirectly predict cancer related fatigue through mental resilience and has a mediating effect.It is suggested that nursing staff should pay more attention to intervene in the mental resilience of patients.In order to improve their hope level,reduce the period of cancer-related fatigue.
