摘　　要：目的探讨单次大分割放疗的临床应用、治疗效果以及潜在的挑战。方法以“single high-dose radiation,single-fraction stereotactic ablative body radiotherapy”为英文关键词,检索PubMed数据库2005-2024年发表的相关文献。纳入标准:(1)单次大分割放射治疗相关文献;(2)包括肺癌、寡转移癌、泌尿生殖系统肿瘤单次大分割治疗的研究。排除标准:重复和数据陈旧的文献。最终共纳入文献42篇。结果单次大分割的使用已经扩展到颅外部位,包括肺、泌尿生殖系统和脊柱肿瘤。与多次大分割放疗相比,单次大分割放疗在肺原发性、寡转移性肿瘤及肾细胞癌中具有有效的局部控制和较小的毒性,但在前列腺癌的局部控制中仍存在争议。单次大分割放疗作为新型治疗方式,可以缓解癌痛,提高癌症患者生活质量。在抗肿瘤免疫中,可以增强免疫原性,改变肿瘤微环境,增强免疫细胞活性,从而降低局部复发,延长患者生存期。结论单次大分割放疗技术广泛应用于肺癌、寡转移癌和泌尿生殖系统肿瘤等各个方面,其局部控制率较好且毒性较低。其在抗肿瘤免疫方面也发挥着巨大作用,但具体作用机制尚不清楚。Objective To explore the clinical application,treatment efficacy,and potential challenges of single dose fractionated radiotherapy.Methods Relevant literature was searched in PubMed database from 2005to 2024,with"single high-dose radiation,singlefraction stereotactic ablative body radiotherapy"as the keywords.Inclusion criteria:(1)Literature related to single high-dose radiation therapy;(2)Studies on single high-dose radiation therapy for lung cancer,oligometastatic cancer,and genitourinary tumors.Exclusion criteria:Duplicate and outdated literature.A total of 42articles were ultimately included in the literature.Results The use of single major segmentation has been extended to extracranial areas,including lung,urogenital system,and spinal tumors.Compared with multiple fractionated radiotherapy,single fractionated radiotherapy has effective local control and less toxicity in primary and oligometastatic lung tumors and renal cell carcinoma,but there is still controversy in the local control of prostate cancer.Single dose fractionated radiotherapy,as a new treatment method,can alleviate cancer pain and improve the quality of life of cancer patients.In anti-tumor immunity,it can enhance immunogenicity,change the tumor microenvironment,enhance immune cell activity,thereby reducing local recurrence and prolonging patient survival.Conclusions Single dose large segment radiotherapy technology is widely used in various fields such as lung cancer,oligometastatic cancer,and urogenital system tumors,with good local control rate and low toxicity.It also plays a significant role in anti-tumor immunity,but the specific mechanism of action is still unclear.
