摘 要:目的探讨超声造影技术在卵巢肿瘤患者中的应用及对良性、恶性肿瘤性质的临床鉴别诊断效能。方法选取58例卵巢肿瘤患者,根据检查方式不同分为对照组及研究组。对照组(29例)接受普通超声诊断,研究组(29例)接受超声造影诊断。以手术病理学诊断结果为“金标准”,比较两组患者卵巢肿瘤良性、恶性的诊断效率,同时对研究组患者良性、恶性肿瘤的血流阻力指数、增强峰值时间、开始增强时间、增强曲线下面积、增强峰值速度进行记录,比较分析良性、恶性卵巢肿瘤的超声造影特征。结果研究组良性、恶性肿瘤诊断准确率分别为86.67%、92.86%,均高于对照组的52.63%、50.00%,差异有统计学意义(P<0.05)。研究组中,恶性肿瘤血流阻力指数低,增强峰值时间、开始增强时间短,增强曲线下面积大,增强峰值强度高,与良性肿瘤指标比较,差异有统计学意义(P<0.05)。结论超声造影技术对卵巢肿瘤良性和恶性的诊断鉴别作用比较突出,可提供多项超声造影特征,帮助临床医师准确分析肿瘤性质。Objective To investigate the application of contrast-enhanced ultrasound in patients with ovarian tumors and its efficacy in differentiating the nature of benign and malignant tumors.Methods 58 patients with ovarian tumor were selected and were divided into a control group and a study group according to different examination methods.The control group(29 cases)received conventional ultrasound diagnosis,and the study group(29 cases)received contrast-enhanced ultrasound diagnosis.Using surgical pathological diagnosis results as the gold standard,the diagnostic efficiency of benign and malignant ovarian tumors in the two groups was compared.Meanwhile,the blood flow resistance index,peak enhancement time,start enhancement time,area under enhancement curve,peak enhancement velocity of benign and malignant ovarian tumors were recorded in the study group,and the contrast-enhanced ultrasound characteristics of benign and malignant ovarian tumors were compared.Results The diagnostic accuracy rates of benign and malignant tumors in the study group were 86.67% and 92.86%,respectively,higher than 52.63% and 50.00% in the control group(P<0.05).In the study group,the blood flow resistance index of malignant tumors was lower,the peak time of enhancement and the beginning time of enhancement were shortor,the area under the enhancement curve was larger,and the peak speed of enhancement was higher,and the differences were statistically significant compared with those of benign tumors(P<0.05).Conclusion Contrast-enhanced ultrasound technique is effective in the diagnosis and differentiation of benign and malignant ovarian tumors,and can provide a number of contrast-enhanced ultrasound features to help clinicians accurately analyze tumor properties.
