摘　　要：目的分析TurboHawk斑块旋切联合药物涂层球囊扩张术治疗股腘动脉闭塞的临床疗效。方法回顾性分析2021年1月至2024年3月河北省人民医院和石家庄市中医院收治的102例股腘动脉闭塞症患者资料,其中54例行TurboHawk斑块旋切联合紫杉醇药物涂层球囊治疗的患者为联合组,48例只进行药物涂层球囊治疗的患者为对照组,对比两组术后1年的一期通畅率、二期通畅率、踝肱指数和间歇性跛行距离等指标。结果手术技术成功率100%。联合组发生远端栓塞2例,及时通过腔内治疗缓解。围手术期未出现心脑血管意外和死亡事件。联合组术后6、12个月的踝肱指数、间歇性跛行距离均高于对照组,差异有统计学意义(P<0.05)。联合组、对照组术后12个月的一期通畅率分别为83.3%(45/54)、72.9%(35/48),差异无统计学意义(P=0.202);二期通畅率分别为94.4%(51/54)、75.0%(36/48),差异有统计学意义(P=0.006)。结论TurboHawk斑块旋切联合药物涂层球囊扩张术治疗股腘动脉闭塞安全有效,可明显改善患者血管通畅率。Objective To analyze the clinical efficacy of TurboHawk plaque rotation combined with drug-coated balloon angioplasty in the treatment of femoropopliteal artery occlusion.Methods The clinical data of 102 patients with femoropopliteal artery occlusion admitted to Hebei General Hospital and Shijiazhuang Hospital of Traditional Chinese Medicine from January 2021 to March 2024 were analyzed retrospectively.Among them,54 patients who underwent TurboHawk plaque rotation combined with paclitaxel drug-coated balloon angioplasty were selected as the combined group,and 48 patients who only underwent drug-coated balloon treatment were selected as the control group.The two groups were compared in terms of primary and secondary patency rate,ankle-brachial index and intermittent claudication distance at 1 year after surgery.Results The technical success rate of the operation was 100%.Two cases of distal embolism occurred in the combined group,which were relieved by endovascular treatment in time.No cardiovascular,cerebrovascular accidents or deaths occurred during the perioperative period.The ankle-brachial index and intermittent claudication distance of the combined group at 6 and 12 months after surgery were higher than those of the control group,and the difference was statistically significant(P<0.05).The primary patency rates of the combined group and the control group at 12 months after surgery were 83.3%(45/54)and 72.9%(35/48),respectively,with no statistically significant differences(P=0.202),and the secondary patency rates were 94.4%(51/54)and 75.0%(36/48),respectively,with statistically significant differences(P=0.006).Conclusion TurboHawk plaque rotation combined with drug-coated balloon angioplasty is safe and effective for the treatment of femoropopliteal artery occlusion,which can significantly improve the vascular patency of patients.
