摘  要:目的探讨利用MRS技术分析偏头痛脑内皮层γ-氨基丁酸(GABA)-谷氨酸(Glu)的改变及其在偏头痛发作中的作用。方法选取19例偏头痛患者为偏头痛组,另选23例性别年龄匹配的健康志愿者为正常对照组(NC组),采集前扣带回及后扣带回的MRS数据并分析GABA、Glu等神经代谢物水平,比较偏头痛组与NC组的GABA、Glu水平差异,并分析偏头痛组发作期神经代谢物水平较发作间期的改变。结果1)与NC组比较,偏头痛发作期前、后扣带回的GABA水平升高,发作间期前扣带回GABA水平有增高趋势,但差异无统计学意义(P>0.05);2)在动态比较中,偏头痛发作期前扣带回GABA水平较发作间期增高。结论偏头痛患者脑内GABA水平的增加与头痛发作有关,这可能与偏头痛的发作通过GABA驱动的抑制调节来维持兴奋-抑制稳态有关。Objective To investigate the alteration of GABA-Glu in migraine and its role in the onset of attacks using MRS technology.Methods This study enrolled 19 migraine patients and 23 sex-and age-matched normal controls and obtained the anterior and posterior cingulate gyrus metabolite concentrations using MRS.We compared the differences in neurometabolite levels between the patient group and the normal control group,and then,analyzed the alterations in the neurometabolite levels during the ictal period compared with the interictal period in the patient group.Results 1)Compared with the normal control group,GABA levels in the anterior and posterior cingulate gyrus were elevated during migraine attacks,and there was a tendency for GABA levels in the anterior cingulate gyrus to increase during the interictal period,but there was no statistical significance(P>0.05);2)In the dynamic comparisons,the GABA levels in the anterior cingulate gyrus were increased in the migraine ictal period compared with the interictal period.Conclusion Increased GABA concentrations in the brain of migraine patients are associated with headache attacks,which may be related to the maintenance of excitatory-inhibitory homeostasis through GABA-driven inhibitory regulation of migraine attacks.
