棕榈酸帕利哌酮及和奥氮平对男性精神分裂症患者的性欲及性激素水平的影响

摘　　要：目的 探讨棕榈酸帕利哌酮及奥氮平对男性精神分裂症的性欲及对性激素的影响。方法 选择2021年1月~2023年1月150例男性精神分裂症患者为研究对象,随机分为奥氮平组与棕榈酸帕利哌酮组,各75例,两组疗程均为3个月。治疗前及治疗3个月后,记录阳性与阴性症状量表(Positive and Negative Syndrome Scale,PANSS)、男子13项目自测性欲量表、精神药物相关性功能障碍问卷(psychotropic drug-related Sexual Dysfunction Questionnaire,PRSexDQ)评分及睾酮、催乳素水平。结果 治疗后,两组PANSS、男子13项目自测性欲量表评分均下降,PRSexDQ评分均上升(P<0.05),但奥氮平组男子13项目自测性欲量表评分高于帕利哌酮组,PRSexDQ评分高于帕利哌酮组(P<0.05)。治疗后,两组催乳素水平均上升(P<0.05),但两组间比较无显著差异(P>0.05)。结论 棕榈酸帕利哌酮及奥氮平在男性精神分裂症中均有较好疗效,对性激素均有影响,但奥氮平对患者性欲的影响更小,而引起的性功能障碍更明显。Objective To investigate the effect of Paliperidone palmitate and olanzapine on sexual desire and sex hormone in male schizophrenia.Methods A total of 150 male patients with schizophrenia from January 2021 to January 2023 were selected as the study subjects and were randomly divided into Olanzapine group and Paliperidone group,with 75 cases in each group.The paliperidone group was given paliperidone palmitate,and the olanzapine group was given olanzapine.Both groups were treated for 3 months.Before treatment and 3 months after treatment,the levels of score of the Positive and Negative Syndrome Scale(PANSS),13 items of male self-rated sexual desire scale,psychotropic drug-related Sexual Dysfunction Questionnaire(PRSexDQ)and testosterone and prolactin were recorded.Results After treatment,the scores of PANSS and 13 items of male self-rated sexual desire scale decreased in both groups,and the scores of PRSexDQ increased in both groups(P< 0.05),but the scores of 13 items of male self-rated sexual desire scale in Olanzapine group were higher than those in Paliperidone group,and the scores of PRSexDQ were higher than those in Paliperidone group(P< 0.05).After treatment,the level of prolactin increased in both groups(P<0.05),but there was no significant difference between the two groups(P> 0.05).Conclusion Both Paliperidone palmitate and olanzapine have good curative effect in male schizophrenia and have effects on sex hormones,but olanzapine has less effect on sexual desire and Paliperidone causes greater sexual dysfunction.
