双相情感障碍患者攻击行为发生情况及危险因素分析

摘　　要：目的 探讨双相情感障碍(Bipolar disorder,BD)的攻击行为发生情况及危险因素。方法 纳入2020年6月~2023年8月105例BD患者,调查是否具有攻击行为并以此分为观察组与对照组,比较两组居住地、病程等资料,并纳入差异显著的资料进行多因素分析。结果 105例患者当中41例具有攻击行为;两组疾病类型、病程、负性事件、自知力、独居、社会支持、自杀或自残史差异具有统计学意义(P<0.05),两组居住地、年龄、教育年限、婚姻、性别差异无统计学意义(P>0.05);多因素分析表明,负性事件、自知力损害、独居、社会支持低水平、自杀或自残史是BD攻击行为的危险因素。结论 BD患者的攻击行为较为常见,负性事件、自知力损害、独居等可能会增加攻击行为发生风险,应注意预防。Objective To investigate the occurrence and risk factors of aggressive behavior in bipolar disorder(BD).Methods A total of 105 cases of BD patients from June 2020 to August 2023 were included.Investigate whether there is aggressive behavior and use this to divide into observation and control groups.Compare the information of the two groups,such as residence and disease duration,and include the information with significant differences for multifactorial analysis.Results Forty-one of the 105 patients had aggressive behavior.Differences in disease type,disease duration,negative events,self-awareness,living alone,social support,and history of suicide or self-injury between the two groups were statistically significant(P<0.05).Differences in place of residence,age,years of education,marriage,and gender between the two groups were not statistically significant(P>0.05).Multifactorial analysis showed that negative events,impairment of self-knowledge,living alone,low level of social support,and history of suicide or self-harm were risk factors for aggressive behavior in BD.Conclusion Aggressive behavior is more common in patients with BD,and negative events,impaired self-awareness,and living alone may increase the risk of aggression and should be prevented.
