自我管理效能在恶性血液病患者自我感受负担与疾病获益感之间的中介作用

摘　　要：目的 探讨恶性血液病患者自我管理效能在自我感受负担与疾病获益感的中介效应,以深入分析恶性血液病患者自我感受负担与疾病获益感之间的关系。方法 在2020年7月~2023年6月期间,选取恶性血液病患者105例,采用自我感受负担量表(Self-perceived burden scale,SPBS)、照顾者疾病获益感量表(Caregiver Disease Benefit Scale,BFS)、中文版自我管理效能量表(Cancer self-management efficacy scale,SUPPH)评估自我感受负担、疾病获益感、自我管理效能,分析恶性血液病患者SPBS、BFS、SUPPH的相关性,评估自我管理效能在自我感受负担与疾病获益感的中介效应。结果 恶性血液病患者SPBS、BFS、SUPPH总分分别为(34.15±6.91)分、(67.14±11.09)分、(71.71±12.43)分。经Pearson分析,SPBS各维度评分及总分与BFS各维度评分及总分呈负相关(P<0.05)。使用Bootstrap抽样检验法进行中介效应研究,SPBS总分=>SUPPH总分=>BFS总分,此条中介路径存在效应。结论 在恶性血液病患者中,其具有较低的自我管理效能,是疾病获益感和自我感受负担的中介作用,提高自我管理效能,可以减轻降低自我感受负担,使得患者拥有较高的疾病获益感。Objective To explore the mediating effect of self-management efficacy of patients with malignant hematological diseases on self-perceived burden and disease benefit,In order to deeply analyze the relationship between the self-perceived burden and the sense of disease benefit in patients with hematological malignancies.Methods From July,2020 to June,2023,105 patients with hematological malignancies were selected.The Chinese version of selfperceived burden(SPBS),caregivers' sense of benefit from illness(BFS)and self-management efficacy(SUPPH)were used to evaluate their self-perceived burden,sense of benefit from illness and self-management efficacy,and the SPBS and BFS of patients with hematological malignancies were analyzed.Results The total scores of SPBS,BFS and SUPPH in patients with hematological malignancies were(34.15±6.91),(67.14±11.09)and(71.71±12.43)respectively.Pearson analysis showed that the scores and total scores of SPBS were negatively correlated with those of BFS(P<0.05).The Bootstrap sampling test method is used to study the mediation effect.The total score of SPBS = >total score of SUPPH = > total score of BFS,and this mediation path has an effect.Conclusion The self-management efficacy of patients with malignant hematological diseases is low,which mediates the sense of benefit from the disease and the burden of self-feeling.Therefore,it is necessary to improve the self-management efficacy of patients,so as to reduce the burden of self-feeling and improve the sense of benefit from the disease.
