延续性护理模式在伴自杀意念青少年抑郁障碍患者中的应用效果

摘　　要：目的:分析延续性护理模式在伴自杀意念青少年抑郁障碍患者中的应用效果。方法:选取2023年2月—2024年2月于南通市第四人民医院出院的80例伴自杀意念的青少年抑郁障碍患者为研究对象。采用随机数字表法分为对照组(40例)和观察组(40例),对照组接受常规护理,观察组在常规护理的基础上实施延续性护理模式,均干预6个月。通过17项汉密尔顿抑郁量表(17-item Hamilton rating scale for depression,HAMD-17)、14项汉密尔顿焦虑量表(14-item Hamilton anxiety scale,HAMA-14)、斯奈思-汉密尔顿快感量表(Snaith-Hamilton pleasure scale,SHAPS)、Beck自杀意念量表(Beck scale for suicide ideation-Chinese version,BSI-CV)、生活满意度量表(satisfaction with life scale,SWLS)、服药依从性量表(morisky medication adherence scale-8,MMAS-8)及功能大体评定量表(global assessment function,GAF)比较两组患者在接受干预前后的抑郁症状、焦虑症状、快感缺失症状、自杀意念、生活满意度、服药依从性以及个体功能的变化。结果:干预后,观察组HAMD-17、HAMA-14、SHAPS、BSI-CV评分均低于对照组(均P<0.05);观察组SWLS、MMAS-8、GAF评分均高于对照组(均P<0.05)。结论:延续性护理模式能显著改善伴自杀意念的青少年抑郁障碍患者的抑郁、焦虑和快感缺失症状,提高患者的生活满意度、个体功能及服药依从性,并在降低自杀意念方面发挥积极作用。Objective:To analyze the application effect of a continuity care model on depression in adolescents patients with suicidal ideation.Methods:Eighty adolescents with suicidal ideation who were discharged from the Nantong Fourth People's Hospital during February 2023 to February 2024 were selected as the research subjects.The patients were divided into a control group(40 cases)and an observation group(40 cases)by using a random number table method,where the control group received routine care,while the observation group was given a continuity care model based on routine care,and both groups were intervened for 6 months.The assessment tools included the 17-item Hamilton rating scale for depression(HAMD-17),the 14-item Hamilton anxiety scale(HAMA-14),the Snaith-Hamilton pleasure scale(SHAPS),Beck scale for suicide ideation-Chinese version(BSI-CV),the satisfaction with life scale(SWLS),the morisky medication adherence scale-8(MMAS-8),and the global assessment function(GAF).The changes in depressive symptoms,anxiety symptoms,pleasure deficit symptoms,suicidal ideation,life satisfaction,medication adherence,and individual function before and after the intervention in the two groups were compared by using these assessment tools.Results:After the intervention,the scores of HAMD-17,HAMA-14,SHAPS,and BSI-CV in the observation group were lower than those in the control group(all P<0.05);the scores of SWLS,MMAS-8,and GAF in the observation group were higher than those in the control group(all P<0.05).Conclusions:The continuity care model can significantly improve depression,anxiety and pleasure deficit symptoms in adolescent depression patients with suicidal ideation,improve patients'life satisfaction,individual function and medication adherence,and play a positive role in reducing suicidal ideation.
