腹泻病例致泻性大肠埃希氏菌感染状况及耐药性分析

摘  要:目的:分析2019—2021年延庆区腹泻病例致泻性大肠埃希氏菌感染状况和耐药性。方法:现收集2019年1月—2021年12月延庆区医院内收治的腹泻患者肛拭子标本和粪便标本,进行荧光定量PCR检测,鉴定标本中致泻性大肠埃希氏菌的分型,对致泻性大肠埃希氏菌的耐药性进行分析。结果:2019—2021年共计1112例样本,肠集聚性黏附性大肠埃希菌(Enteroaggregative Escherichia coli,EAEC)检出率为5.57%,肠道致病性大肠埃希菌(Enteropathogenic Escherichia coli,EPEC)检出率为1.80%、产肠毒素性大肠埃希菌(Enterotoxigenic Escherichia coli,ETEC)检出率为1.35%,肠道侵袭性大肠埃希菌(Enteroinvasive Escherichia coli,EIEC)检出率为0.27%,未检测出肠出血性大肠埃希菌(Enterohemorrhagic Escherichia coli,EHEC);2019年6月检出率最高,2020年7月、8月检出率最高,2021年7月检出率最高;2019年36株致泻性大肠埃希氏菌(Diarrheagenic Escherichia coli,DEC)对磺胺异噁唑耐药率(91.7%)最高,2020年32株DEC对磺胺异噁唑的耐药率(100.0%)最高,2021年32株DEC对萘啶酸的耐药率(75.0%)最高。结论:2019—2021年延庆区腹泻病例致泻性大肠埃希氏菌感染状况基本稳定,且其耐药性方面获得了较好的控制,临床效果比较明显。Objective:To analyze the infection status and drug resistance of diarrhea causing Escherichia coli in Yanqing district from 2019 to 2021.Method:Anal swab specimens and fecal specimens of diarrhea patients admitted to Yanqing district hospital from January 2019 to December 2021 were collected for fluorescence quantitative PCR detection to identify the types of diarrhea causing Escherichia coli in the specimens and analyze the drug resistance of diarrhea causing Escherichia coli.Results:A total of 1112 samples were collected from 2019 to 2021.The detection rate of enteroaggregative Escherichia coli(EAEC)was 5.57%,the detection rate of enteropathogenic Escherichia coli(EPEC)was 1.80%,the detection rate of enterotoxic Escherichia coli(ETEC)was 1.35%,and the detection rate of enteroinvasive Escherichia coli(EIEC)was 0.27%,and no enterohemorrhagic Escherichia coli(EHEC)was detected.The detection rate was the highest in June 2019,July and August 2020,and July 2021.In 2019,36 strains of diarrheagenic Escherichia coli(DEC)had the highest resistance rate to sulfamethoxazole(91.7%),in 2020,32 strains of DEC had the highest resistance rate to sulfamethoxazole(100.0%),and in 2021,32 strains of Dec had the highest resistance rate to nalidixic acid(75.0%).Conclusion:The status of diarrhea causing Escherichia coli infection in diarrhea cases in Yanqing district from 2019 to 2021 is basically stable,and its drug resistance has been well controlled,and the clinical effect is relatively obvious.
