摘 要:目的:研究微种植体联合片段弓技术压低上颌伸长磨牙的临床疗效以及牙根吸收的发生情况。方法:选择需压低的上颌伸长磨牙患者24例,按照第一磨牙、第二磨牙分为A、B两组,每组各12例。在治疗前(T0)、治疗后(T1)分别拍摄锥形束CT(cone beam computed tomography,CBCT),测量压低量以及牙根长度的变化量。使用CBCT结合Mimics软件对治疗前后伸长磨牙进行三维重建并计算牙体积及牙表面积的减少量。运用SPSS 23.0软件进行统计学分析。结果:A、B两组所有磨牙均被有效压低,平均压低量分别为(3.03±0.17)mm、(3.06±0.21)mm;牙根长度的平均减少量为(0.46±0.11)mm、(0.47±0.13)mm;牙体积减少量分别为(57.60±3.79)mm^(3)、(57.10±3.85)mm^(3);牙表面积减少量分别为(30.60±4.42)mm^(2)、(32.38±3.04)mm^(2);牙体积减少量与压低量有显著相关性(P<0.05)。结论:微种植体联合片段弓技术压低上颌伸长磨牙治疗效果显著,但并非绝对压低,部分由牙根吸收引起。Objective:To investigate the clinical effect of micro-implant combined with segmental arch technique in intrusion of maxillary extended molars and the incidence of root resorption.Methods:Twenty-four patients with maxillary extended molars requiring intrusion were divided into groups A and B according to the first molar and second molar,with 12 patients in each group.Cone beam computed tomography(CBCT)was taken before treatment(T0)and after treatment(T1)to measure the pressure and root length change.CBCT combined with Mimics software(Materialise’s Interactive Medical Image Control System)was used to reconstruct the elongated molars before and after treatment.The reduction of tooth volume was calculated.Results:All the molars in group A and group B were effectively depressed,and the average depressions were(3.03±0.17)mm and(3.06±0.21)mm,respectively.The average root length reduction was(0.46±0.11)mm and(0.47±0.13)mm,respectively.The tooth volume loss was(57.60±3.79)mm^(3) and(57.10±3.85)mm^(3),respectively.The reduction of tooth surface area was(30.60±4.42)mm^(2) and(32.38±3.04)mm^(2),respectively.Teeth were significantly correlated with the amount of volume reduction and lower(P<0.05).Conclusion:Micro-implant combined with segmental arch intrusion is effective in the treatment of maxillary extended molars,but the intrusion is not absolute,which is partly caused by root resorption.
