乳房下象限肿瘤保乳整形手术中应用乳房下脂肪筋膜瓣修复的效果

摘　　要：目的探讨乳房下象限肿瘤保乳整形手术中应用乳房下脂肪筋膜瓣的效果。方法选取2021年9月至2023年9月乐山市人民医院行乳房下象限肿瘤保乳整形手术患者96例,根据乳房缺损修复方法分为观察组(n=52)和对照组(n=44),2组均给予根治性手术,其中观察组给予乳房下脂肪筋膜瓣修复,对照组给予胸外侧脂肪筋膜瓣修复。观察2组手术相关指标、乳房外观差异、并发症等。结果在手术时间、术中出血量、术后引流时间、术后引流量、术后局部复发及远处转移发生率比较中,观察组和对照组差异无统计学意义(P>0.05)。观察组术后乳房外观优于对照组(P<0.05),其优良率达到98.08%。观察组术后1年乳腺癌患者生命质量测定量表(FACT-B)评分为(105.55±10.03)分,明显高于对照组的(94.44±11.12)分(P<0.05)。观察组并发症总发生率为3.85%,明显低于对照组(20.45%)(P<0.05)。结论乳房下象限肿瘤保乳整形手术应用乳房下脂肪筋膜瓣修复,术后乳腺美容效果好,手术时间及手术并发症无明显增加。Objective To explore the effect of applying the inframammary fat fascia flap in breast-conserving plastic surgery for tumors in the lower quadrant of the breast.Methods Ninety-six patients who underwent breast-conserving plastic surgery for lower quadrant tumors in Leshan People's Hospital from September 2021 to September 2023 were selected and divided into an observation group(n=52)and a control group(n=44)according to the method of breast defect repair.Both groups underwent radical surgery.The observation group was given repair with the inframammary fat fascia flap,while the control group was given repair with the lateral thoracic fat fascia flap.The surgery-related indicators,differences in breast appearance,and complications of the two groups were observed and compared.Results In the comparisons of operation time,intraoperative blood loss,postoperative drainage time,postoperative drainage volume,incidence rates of local recurrence and distant metastasis,statistical analysis showed that there was no significant difference between the observation group and the control group(P>0.05).The postoperative breast appearance of the observation group was better than that of the control group(P<0.05),with an excellent and good rate of 98.08%.The score of the Functional Assessment of Cancer Therapy-Breast(FACT-B)of breast cancer patients in the observation group one year after surgery was(105.55±10.03)points,which was significantly higher than that of the control group[(94.44±11.12)points](P<0.05).The total incidence rate of complications in the observation group was 3.85%,which was significantly lower than that of the control group(20.45%)(P<0.05).Conclusion The application of the inframammary fat fascia flap in breast-conserving plastic surgery for lower quadrant tumors has good postoperative breast cosmetic effects,without a significant increase in operation time and surgical complications.
