名中医李果烈基于“治风先治血”理论分期论治帕金森病经验

摘　　要：目前,临床上帕金森病大多从脏腑辨证、阴阳辨证等角度分析疾病发生发展的病机演变规律。为了不断发展完善帕金森病的中医辨证论治体系,李果烈教授结合自己多年的临床经验,以“治风先治血,血行风自灭”为理论基础,从全新的视角基于“血的转化”详细论述了帕金森病的病机演变规律,认为帕金森病的进展过程就是“风动”不断演变发展的过程,“风动”的变化过程就是“血”的转化过程,其发展大致沿“血热”-“血瘀”-“血虚”的方向不断进展。因此,李老在帕金森病的早中晚期调节阴阳的同时分别配伍凉血、活血、补血的理血药物。核心治法为早期滋阴清热,凉血息风;中期阴阳双补,化瘀止颤;晚期温补肾阳,补益气血。李果烈教授根据临床实际情况辨证论治,将该理论运用于临床实践中,获效明显。At present,most clinical Parkinson’s disease from the perspective of zang-fu syndrome differentiation,Yin and Yang syndrome differentiation and so on to analyze the pathogenesis and evolution of the disease.In order to continuously develop and improve the dialectical treatment system of traditional Chinese medicine for Parkinson’s disease,Professor LI Guolie combined his many years of clinical experience,based on the theory of“treating the blood first,and eliminating the blood flow wind itself”,discussed in detail the pathogenesis of Parkinson’s disease from a new perspective based on“blood transformation”,and believed that the progression of Parkinson’s disease is the continuous evolution and development of“wind movement”.The change process of“wind”is the transformation process of“blood”,and its development roughly progresses along the direction of“blood heat”-“blood stasis”-“blood deficiency”.Therefore,LI Guolie in the early,middle and late stages of Parkinson’s disease to regulate Yin and Yang at the same time,respectively,with cooling blood,promoting blood,supplementing blood drugs.The core treatment is early nourishing Yin and clearing heat,cooling blood and calming wind;metaphase Yin and Yang double supplement,remove stasis and stop fibrillation;late warm tonifying kidney Yang,tonifying qi and blood.According to the clinical situation,Professor LI Guolie applied the theory to clinical practice,and obtained obvious effect.
