晚期结肠癌患者希望水平调查分析及其影响因素

摘　　要：目的探讨晚期结肠癌患者希望水平调查及其影响因素。方法选择2021年12月—2023年12月本院收治的79例晚期结肠癌患者作为研究对象。收集资料包括性别、年龄、BMI、文化程度、家族史、宗教信仰、民族、家庭月收入、主观幸福感、应对方式、负性情绪、恐惧疾病进展。采用希望水平量表(HHI)评价患者希望水平;应用社会支持状况(SSRS)量表评定患者社会支持状态。结果发放79份问卷,回收问卷79份,有效问卷76份,最终纳入研究患者76例。晚期结肠癌患者HHI量表总分(28.21±6.53)分,处于中等希望水平。不同性别、年龄、BMI、家族史和宗教信仰患者HHI量表评分比较,差异无统计学意义(P>0.05);小学及以下文化水平患者HHI量表评分低于初中~高中与高中以上水平患者(P<0.05);家庭月收入<3000元患者HHI量表评分低于3000~5000元与≥5000元患者(P<0.05);主观幸福感不强患者HHI量表评分低于主观幸福感强烈患者(P<0.05);消极应对方式患者HHI量表评分低于积极应对方式患者(P<0.05);负性情绪患者HHI量表评分低于无负性情绪患者(P<0.05);恐惧疾病进展患者HHI量表评分低于无恐惧疾病进展患者(P<0.05)。家庭月收入<3000元、主观幸福感不强、消极应对方式、负性情绪和恐惧疾病进展为影响希望水平独立危险因素。结论晚期结肠癌患者处于中等希望水平,且受多因素影响,其中家庭月收入低、主观幸福感不强、应对方式消极、有负性情绪、恐惧疾病进展为影响希望水平独立危险因素。Objective:To investigate the hope level and its influencing factors of patients with advanced colon cancer.Methods The paper chose 79 patients with advanced colon cancer from December 2021 to December 2023 as study subjects,collected data of gender,age,BMI,educational level,family history,religious beliefs,ethnicity,monthly household income,subjective well-being,coping style,negative emotions and fear of disease progression.The hope level was evaluated with(HHI)scale;social support status of patients was evaluated with social support rating scale(SSRS).Results The paper distributed 79 questionnaires,collected 79,76 questionnaires were valid,and 76 cases were included in the study.Total scores of HHI scale of advanced colon cancer patients were(28.21±6.53)points,which was at a moderate hope level.There were no significant difference in HHI scores among different genders,ages,BMI,family history and religious beliefs,(P>0.05).The HHI scores of patients with primary school education and below were lower than those with middle school to high school education and above(P<0.05).The HHI scores of patients with monthly household income less than 3000 were lower than those with 3000-5000 and above 5000(P<0.05).The HHI scores of those with weak subjective well-being were lower than those with strong subjective well-being(P<0.05).The HHI scores of those with negative coping styles were lower than those with positive coping styles(P<0.05).The HHI scores of those with negative emotions were lower than those without negative emotions(P<0.05).The HHI scores of those with fear of disease progression were lower than those without fear of disease progression(P<0.05).Family monthly income<3000,weak subjective well-being,negative coping strategies,negative emotions and fear of disease progression were independent risk factors of hope level.Conclusion Patients with advanced colon cancer have a moderate hope level,which can be influenced by multiple factors,and monthly family income,subjective well-being,coping strategies,negative emo
