摘　　要：目的了解盐城某院抗肿瘤药物使用情况,以促进临床安全合理用药。方法采用回顾性方法,随机抽取某院2022年1—12月肿瘤科、非肿瘤科(放疗科、呼吸科及部分外科)出院患者病历共540例,剔除未使用抗肿瘤药物病历37例,依据科室不同将余下503例使用抗肿瘤药物病历分为两组,一组为肿瘤组(n=327),另一组为非肿瘤组(n=176),结合相关资料,对影响因素进行统计分析。结果肿瘤组与非肿瘤组患者(以下用两组表示)在化疗药单药、化疗药二联及以上和抗肿瘤中药使用上有显著差异,具统计学意义(P<0.05)。在抗肿瘤中药使用上,性别有显著差异,具统计学意义(P<0.05);肿瘤组不合理用药病历数117例(35.77%),主要问题为用法用量不适宜、适应证不适宜、重复用药等;非肿瘤组不合理用药病历数87例(49.43%),主要问题为用法用量不适宜、适应证不适宜、预防用药不规范等。结论抗肿瘤药物在某院使用情况尚可,但仍存在不合理用药现象,尤其非肿瘤组较明显。因此,医院应加强抗肿瘤药物临床使用的管理,定期培训并考核相关医护人员,以促进合理用药,确保肿瘤患者的用药安全。Objective To understand use of anti-tumor drugs in a hospital in Yancheng,to promote safe and rational clinical drug use.Methods The paper chose 540 medical records of discharged patients in oncology department and non oncology departments(radiotherapy department,respiratory department and some surgery departments)in a certain hospital from January to December 2022 randomly with retrospective method,and analyzed 503 medical records statistically with antitumor drugs and influencing factors after excluding 37 medical records withnot using anti-tumor drugs.Results There was significant difference in use of chemotherapy monotherapy,chemotherapy combination therapy or above,and anti-tumor traditional Chinese medicine between oncology and non oncology patients(hereinafter referred to as two groups),(P<0.05).There was significant gender difference in use of anti-tumor traditional Chinese medicine,with statistical significance(P<0.05).There were 117 cases(35.77%)of irrational drug use in the oncology department,including inappropriate usage and dosage,inappropriate indications,and repeated use of drugs.There were 87 cases(49.43%)of irrational drug use in non oncology departments,including inappropriate usage and dosage,inappropriate indications,and non-standard preventive medication.Conclusion Anti-tumor drug use in a certain hospital is still acceptable,but there are irrational drug use phenomena,especially in non oncology departments.Therefore,hospitals should strengthen management of clinical use of anti-tumor drugs,train and assess relevant medical staff regularly to promote rational drug use and ensure safety of drug use of cancer patients.
