摘  要:目的探讨拇内收肌厚度(adductor pollicis muscle thickness,APMT)测量在脑卒中患者营养评估中的临床价值。方法选择160名住院脑卒中患者作为观察组,收集其健侧APMT、上臂围、皮褶厚度、小腿围的数据。应用主观综合性评估(subjective global assessment,SGA)营养量表进行营养评估,根据SGA评分分为三个亚组,分析APMT与SGA评分等级的相关性。运用ROC曲线描述SGA与患者健侧的APMT、小腿围、上臂围、皮褶厚度的关系。结果APMT随SGA评分等级增高而减少,P<0.001。ROC曲线表明,APMT较传统指标在脑卒中患者营养评估中的敏感性更高。结论APMT敏感性高、简便、有效,可应用于脑卒中患者的营养评估。Objective To investigate the clinical value of adductor pollicis muscle thickness(APMT)in nutritional evaluation of stroke patients.Methods A total of 160 inpatients with stroke were selected for the observation group to collect data of APMT,upper arm circumference,skinfold thickness and calf circumference of the healthy side.The subjective global assessment(SGA)scale was used for nutrition evaluation and they were divided into three groups.The correlation between APMT and SGA was analyzed.The ROC curve was used to describe the relationship between SGA and APMT,calf circumference,upper arm circumference and skinfold thickness of the patients′dominant side.Results The APMT decreased with the increase of SGA rating,P<0.001.The ROC showed that APMT was more sensitive than other traditional indicators in nutritional assessment of stroke patients.Conclusion The measurement of APMT is highly sensitive,simple and effective,and can be used in the nutritional evaluation of stroke patients.
